SALES CONDITIONS AND WARRANTIES


AUTHORIZATION

I hereby authorize the work and/or purchase of goods and services referred to on
this invoice and agree to pay the total amount referred to herein. Where the
work and/or goods and services are covered by an insurance policy, I authorize
Belron Canada Inc. «Belron», for and on my behalf, to seek the full payment of
the amount referred to under «Amount payable by insurer» directly to my
insurance company or its authorize representative. I agree that this request for
payment is made by Belron only to accommodate me and in the event that my
insurance company or its representative fails to authorize payment of such
amount. I agree to pay any unpaid residual amount. Ownership of the goods sold
referred to on this invoice remains with Belron until full payment of this
invoice. I agreed to pay interest at 1.5% par month on overdue accounts.


PERSONAL INFORMATION

At Belron, we are committed to protecting your privacy. You consent to your
personal information being collected and retained in our database, and user by
Belron and disclosed to and used by third parties as required for the purpose of
discharging the work, and processing and managing the account therefore and
related insurance coverage. Belron will not sell trade or rent your personal
information to others. You may contact us directly to review and update your
personal information that we hold.


INSURANCE DECLARATION

I declare the information contained herein to be true and correct to the best of
my knowledge and that the loss or damaged did not occur through any willful act,
procurement means or connivance of the undersigned and the repair or replacement
has been completed to my entire satisfaction and hereby authorize my insurance
company to make payment directly to Belron in full settlement of its obligations
under my policy for said claim.


CREDIT CARD AUTHORIZATION

The issuer of the card identified herein is authorized to pay the amount shown
as total upon proper presentation. I promise to pay such total (together with
any others charges due thereon) subject to and in accordance with the agreement
governing the use of such card.


WARRANTY CONDITION

Subject to the following terms and conditions, this warranty will be honoured at
all Belron locations throughout Canada.

Once the invoice is paid in full, Belron warrants that automobiles glass (or
purchased product) is free from defects in material or workmanship for the
periods indicated on the front of the invoice. HOWEVER, BELRON SHALL NOT BE
LIABLE FOR ANY DAMAGES OR DEFECTS CAUSED BY.

-    IMPROPER USE, NEGLIGENCE, ALTERATION, ACCIDENT, INCIDENT OR ROAD HAZARD,
-    INADEQUATE REPAIR ATTEMPTED OR PERFORMED BY THE CONSUMER OR ANY THIRD
PARTY, OR
-    USE OF THE VEHICLE FOR COMPETITIVE PURPOSES.

Where any automobile glass (or purchased product) fails due to defects in
material or workmanship during normal usage, during the warranty period
specified on the front of the invoice, Belron shall repair or replace the
defective part without charge. If Belron is unable to make the repair or
replacement, Belron will refund the original purchase price of your warranted
product.

To obtain a warranty repair, you must take your vehicle to a Belron location
together with a copy of the original Belron invoice as proof of purchase. NO
REFUND OR CREDIT WILL BE GRANTED FOR ANY WORK OR REPAIRS DONE BY THE CONSUMER OR
ANY THIRD PARTIES.

Belron warranties apply only to complete and proper installation and repair.
WARRANTIES MAY BE REDUCED OR WITHDRAWN WHEN CUSTOMERS ELECT INCOMPLETE OR
IMPROPER INSTALLATION (such as installation over rusting metal, where the
customer declined rust repair). Such details will be noted on the front of
invoice. ALL SEALANT WARRANTIES EXCLUDE SITUATION WHRE LEAKS ARE CAUSED BY
RUSTED METAL AND/OR BODY DETERIORATION.

To the extents permitted by law, Belron’s warranty is non-transferable and
becomes null and void upon a change in ownership of the vehicle repaired or the
product purchased.


LIMITATION OF LIABILITY

THE WARRANTIES DESCRIBED ON THIS INVOICE ARE IN LIEU OF ALL OTHER WARRANTIES OR
CONDITIONS, EXPRESS OR IMPLIED, WRITTEN OR VERBAL, BELRON SHALL NOT BE LIABLE TO
THE PURCHASER FOR ANY CLAIMS REGARDINS FAILURE OR DEFECT OF THE PURCHASED GOODS
OR SERVICES RESULTING IN DIRECT OR INDIRECT, INCIDENTAL OR CONSEQUENTIAL, LOSS
OR DAMAGE, PERSONAL INJURY OR DEATH, IN NO CASE SHALL THE LIABILITY OF BELRON
EXCEED THE PURCHASE PRICE PAID FOR THE GOODS BY THE PURCHASER.

Some provinces do not allow the exclusion or limitation of a warranty or of
incidental or consequential damages, so the above limitation and exclusions may
not apply to you. This warranty gives you specific legal rights and you may also
have additional rights which vary from province to province. The terms and
conditions contained herein do not limit or otherwise diminish any rights to
which you may be entitled under consumer protection legislation in any province.
