 * Have you experienced a disruptive life event such as a break-up or divorce,
   the death of a loved one, or the loss of a job?
 * Do you feel angry, sad, frustrated, overwhelmed, or alone?
 * Do you just want to improve your mental well-being and live a healthier life?

 





  Here's How To Get Started:




STEP 1: REACH OUT


THE CARE JOURNEY BEGINS BY SIGN-UP OR REFERRAL. ONCE CONNECTED WITH CHECK-IN, A
TRAINED "CARE COORDINATOR" WILL BE ASSIGNED TO YOU OR YOUR LOVED ONE WITHIN 72
HOURS. THE CARE COORDINATOR WILL LISTEN, ANSWER QUESTIONS, AND BEGIN THE CARE
PROCESS.


STEP 2: GET CONNECTED


AFTER ADMINISTERING AN INITIAL INTAKE SCREENING, THE CARE COORDINATOR WILL
DEVELOP A PERSONALIZED CARE PLAN, MATCHING YOU OR YOUR LOVED ONE'S UNIQUE NEEDS
TO THE PROGRAMS AND ORGANIZATIONS BEST ABLE TO PROVIDE HELP.


STEP 3: RECEIVE CARE


THE CARE COORDINATOR WILL THEN ASSIST IN SCHEDULING CARE, WITH TREATMENT OPTIONS
RANGING FROM OUTPATIENT AND TELEHEALTH PROGRAMS TO INTENSIVE, IN-PERSON CARE
PROGRAMS. THE CARE COORDINATOR WILL CONTINUE TO SERVE AS AN ADVISOR AND GUIDE
THROUGHOUT THE CARE JOURNEY.
