OVERVIEW


ACTIVITY DESCRIPTION

Providing appropriate care to patients at the end of life, and to their
families, is an important role in a primary care practice. Incorporating
palliative care into a primary care practice has the potential not only of
improving the quality of care delivered to patients, but also of enhancing the
professional satisfaction of physicians. The primary care setting is an
excellent venue for addressing end-of-life care relevant to all patients, and
the primary care provider (PCP) can serve as an advocate for improved care of
the dying by maintaining comfort and optimizing quality of life. This activity
will discuss the role of palliative care in a primary care setting and the
impact it has on quality of life for patients and their families. Patients who
can benefit from palliative care will be identified and strategies to engage
patients in the palliative-care discussion will also be reviewed, along with
resources for both patients and providers that require a higher level of care.

TARGET AUDIENCE

This activity is designed to meet the educational needs of physicians, PAs,
nurses, nurse practitioners, and other HCPs who have an interest in learning
about incorporating palliative care into their primary care practice.


LEARNING OBJECTIVES

At the end of this activity, participants should be able to:

 * Define palliative care and the impact it has on quality of life for patients
   and their families.
 * Describe the characteristics of patients who can benefit from palliative
   care.
 * Identify specific actions PCPs can take to provide primary palliative care.
 * Review tools and strategies to engage patients in palliative-care
   discussions.
 * Identify when additional resources are needed for both patients and providers
   that require a higher level of care.

About the presenters
Caroline DeFilippo, MD, MPH, FACP, is thrilled to be a part of Optum East,
practicing in the same community where she was born and raised. Dr. DeFilippo
graduated from Harvard University in 2001 and then spent one year pursuing
graduate studies in Australia as part of a Rotary Ambassadorial Scholarship.
After working in public health for a number of years focusing on chronic disease
management, she completed her medical degree and master’s degree in public
health at Mount Sinai Hospital in 2009. She was honored for her outstanding
teaching and humanism skills during her training. Her residency in internal
medicine, also at Mount Sinai, focused on primary care. Prior to joining
CareMount, she was in private practice for five years and served as an associate
medical director at WestMed Medical Group. She now serves as a medical director
for population health and adult primary care, continuing to practice medicine
while pursuing her administrative roles. She is passionate about primary care
particularly as it allows for the development of long-term partnerships with
patients and their families. She has also brought the Serious Illness
Conversation training to over 200 providers. 

Kristofer Smith, MD, MPP, as chief medical officer of Landmark Health, leads
efforts to establish a high quality and clinically effective home-based medical
care model for patients with serious illness. He oversees clinical model design,
learning and development, quality of care standards, and clinical improvement
activities. His prior roles include chief clinical officer at Prospero Health, a
home-based palliative care company, president of NaviHealth’s home-based medical
care division and senior vice president of population health at Northwell
Health. He has published papers on advanced-care models for the frail elderly
and regularly gives national talks on health policy, the frail elderly and the
intersection of payment reform, and clinical redesign. He currently serves on
the board of the American Academy of Home Care Medicine. He worked as a
house-call physician for more than a decade, providing primary and palliative
care to the frail homebound elderly.
