FULLSCRIPT BRAND PULSE SURVEY: GIVEAWAY OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN. PURCHASE DOES NOT INCREASE THE CHANCES OF
WINNING.

Natural Partners, LLC (“Fullscript”) is conducting a Fullscript Brand Pulse
Survey (“Survey”) with an incentive prize giveaway (“Giveaway”). Fullscript is
an e-commerce platform that allows patients easy access and ordering of dietary
supplements recommended by their healthcare practitioner. The Survey will be
available from Wednesday, September 21, 2022 at 1:00 p.m. ET to Wednesday
September 28, 2022 at 1:00 p.m. ET.

 


GIVEAWAY OFFICIAL RULES

Incentive Giveaway Prize: There will be one (1) prize available with a retail
value of up to $200.00 at the end of the Giveaway (“Prize”). The Prize will
consist of one (1) USD $200 gift card to Amazon. The Prize is subject to any
terms and conditions which may be contained thereon. The Prize must be accepted
as awarded and has no cash value and is non-transferrable. All prizes will be
awarded. Fullscript reserves the right to substitute an alternate Prize of equal
or greater value if the selected Prize is unavailable at time of award. Any
Prize-related federal, state and/or local taxes shall be the responsibility of
the Winner.

How to Enter: The Giveaway is open to healthcare practitioners , who complete
the Survey and who are legal residents of the USA. To be entered into the
Giveaway, an Entrant must complete the Survey using the link provided in the
email. LIMIT one (1) entry per eligible Entrant, regardless of the number of
Survey responses received from any one Entrant. Use of any robotic, automatic,
programmed, or similar entry method or entering more than the number of times
permitted will void all entries and result in disqualification. You may not
enter more times than indicated by using multiple email addresses, identities,
or devices in an attempt to circumvent the Official Rules

 


TERMS AND CONDITIONS.

 1.  By completing and submitting the Survey, you fully and unconditionally
     agree to and accept these Giveaway Official Rules and all decisions of
     Fullscript, which shall be final in all matters.
 2.  Entries are void where prohibited by law. All federal, state and local laws
     and regulations apply. Fullscript reserves the right to terminate or amend
     this Giveaway at any time and in any way, without prior notice. If, for any
     reason, the Giveaway is not capable of running as originally planned,
     Fullscript reserves the right to cancel the Giveaway and conduct a random
     draw from all eligible entries received as of the cancellation date. The
     Giveaway begins on Wednesday, September 21, 2022 at 1:00 p.m. ET and ends
     on Wednesday September 28 , 2022 at 1:00 p.m. ET.
 3.  Eligibility: Entrants must be at least 18 years of age to enter. Employees
     of Fullscript, its parent or other affiliated companies, and its agents,
     including those living in the same household of Fullscript and its agents,
     are ineligible to participate. No purchase is required and a purchase does
     not enhance your chances of winning.
 4.  Effective Date of Entries: Entries will be effective as of the date and
     time the completed Survey is submitted to Fullscript. All Survey entries
     become the property of Fullscript and will not be acknowledged or returned.
 5.  Winner Selection: The winners will be determined by a random draw from all
     eligible Entrants received (“Winner”). The random draw will be held on or
     about Thursday, September 29, 2022 at 11:00 a.m. ET. By completing the
     Survey and participating in the Giveaway, Entrants consent to the use of
     their name for the administration of the Giveaway or any publicity carried
     out by Fullscript, without further notice or compensation, unless
     prohibited by law.
 6.  The Winners will be notified on Thursday, September 29nd, 2022 by 5:00 p.m.
     ET via the email address used to send the Survey. Failure of the Winner to
     respond to the email message from Fullscript within forty-eight (48) hours
     after the email has been sent will result in forfeiture of the Prize by
     Winner and Fullscript will select a new Winner by way of random draw.
     Winners must meet the eligibility criteria set out in these Giveaway
     Official Rules. All decisions by Fullscript are final and binding in all
     matters relating to the Giveaway.
 7.  Odds of Winning: The odds of winning depend on the number of eligible
     entries received before the Giveaway end date.
 8.  Liability, Indemnification and Release: Fullscript assumes no
     responsibility or liability for (a) any incorrect or inaccurate entry
     information, or for any faulty or failed electronic data transmissions; (b)
     any unauthorized access to, or theft, destruction or alteration of entries
     at any point in the operation of the Survey and Giveaway; (c) any technical
     malfunction, failure, error, delay in operation or communications,
     regardless of cause; (d) inaccessibility or unavailability of any network
     or wireless service, the Internet or website or any combination thereof;
     (e) suspended or discontinued Internet service; or (f) any injury or damage
     to Entrant’s or to any other person’s computer or mobile device which may
     be related to or resulting from any attempt to participate in the Survey
     and Giveaway. By entering this Giveaway, Entrant agrees that they release,
     discharge and hold harmless Fullscript and its affiliates and their
     respective officers, employees, shareholders, directors, members, managers,
     and employees from any and all losses, damages, rights, claims and actions
     of any kind in connection with the Prize or this Giveaway, including
     without limitation, personal injury, death, and property damages and claims
     based on publicity rights, defamation or invasion of privacy (including
     punitive, incidental, and consequential damages). SOME JURISDICTIONS DO NOT
     ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR
     CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU.
 9.  Intellectual Property: All intellectual property, including but not limited
     to trademarks, logos, designs, promotional materials, are owned by
     Fullscript. All rights reserved. Unauthorized copying or use of any of
     Fullscript’s intellectual property without the express written consent of
     Fullscript is strictly prohibited.
 10. Governing Law: All federal and state laws apply. The Giveaway is void where
     prohibited. The Giveaway and all matters arising out of or relating to the
     Giveaway will be governed by, and construed in accordance with, the laws of
     the State of Delaware, USA, without giving effect to any of its conflict of
     laws provisions thereof. Entrant further agrees that any legal suit,
     action, or proceeding arising out of or relating to the Giveaway and these
     Official Rules shall be brought exclusively in the applicable federal or
     state courts located in Delaware.
 11. Winners List: For the name of the prize winner, send a self-addressed,
     stamped envelope to: Fullscript Brand Pulse Survey, 317 S 48th St, Phoenix,
     AZ 85034, United States.
