UN MÉDECIN ARNAQUE 77 000€ AVEC DES PATIENTS FANTÔMES
Avec près de 80 000€ extorqués à la CPAM et la MSA, c’est une arnaque d’ampleur
qui a conduit un médecin d’une commune proche de Lannion au tribunal. C’est en
janvier 2020 que la CPAM (Caisse primaire d’Assurance Maladie) des Côtes-d’Armor
a saisi le procureur de la République de Saint-Brieuc pour des faits de
suspicions d’escroquerie et d’usage de faux. C’est l’activité atypique de ce
médecin qui a mis en alerte les agents de cet organisme. En effet, ce praticien
avait parmi sa patientèle sept fois plus de patients bénéficiant de la CMU-C
(Couverture Maladie Universelle Complémentaire) que les autres médecins
généralistes du département. De plus, contrairement à la quasi-totalité de ses
confrères, ce médecin ne « télétransmettait » pas à la CPAM ou à la MSA les
factures de façon quotidienne. Il envoyait régulièrement un même jour, plusieurs
factures concernant un même patient, pour des consultations à des dates
différentes.
En un peu plus de deux ans, 65 actes ont été facturés alors que le médecin était
en villégiature. Les enquêteurs ont également entendu une vingtaine de patients
et les déclarations sont accablantes pour ce médecin. Même si certains le
décrivent comme un « bon docteur », les pratiques frauduleuses de l’intéressé
font peu de doutes.
Le médecin disait que le boîtier ne fonctionnait pas bien, il passait donc la
carte à deux reprises. Ce qui engendrait automatiquement une seconde facture. Un
patient peut même préciser que cela s’est passé à six reprises le concernant.
Plusieurs patients n’ont aucun souvenir d’une consultation à certaines dates,
pour une autre cela est matériellement impossible, ne se trouvant pas dans la
région à la date indiquée. Pour d’autres encore, plusieurs consultations dans le
même mois alors qu’ils sont certains de n’avoir vu leur médecin qu’une seule
fois. L’enquête a également mis en évidence, une différence significative entre
le nombre de factures transmises aux organismes de santé et le nombre de
consultations recensées sur l’agenda du praticien. C’est environ 250 factures
par trimestre en surplus entre février 2017 et avril 2019. Interrogé à la barre

A 63-year-old man presented with 2 years of history of cough and bloody sputum.
Fiber optic bronchoscopy that was performed through the endotracheal tube showed
a cavity in the right upper lobe apical segment bronchus. A spherical mass
covered with a coating of white pus can be seen when the bronchoscopy enters
inside the cavity.

9 MONTH OLD GIRL WITH BLUNT FORCE HEAD TRAUMA
A 9 month old is brought to the ER by her mother after launching out of a faulty
highchair. According to mom, she fell from the 5 foot chair and landed on her
forehead. She immediatly began crying and developed an area of abou 5 cm at the
area of imact. In the ambulance on the way to the hospital the mother notices
the child is not behaving like herself (she normally has bad stranger fright and
now is very quiet and passive with the medics). Physical and neurological exams
with reflexes are all within normal range for her age. She is 70th percentile
for weight and 82nd percentile for height, was born vaginally without
complications at 39 weeks and has no other relevant medical history.
The mother is very anxious about her child and requests imaging. How would you
proceed?

38 YEAR OLD WOMAN WITH E. COLI UTI
I have a 38 year old female patient with an anaphylactic allergy to
floroquinolones, who is otherwise healthy but she frequently gets UTIs. After a
day of her classic UTI symptoms she did a urine culture. Initial UA did not show
signs of infection but culture came back positive for E. coli. She recieved the
results over the weekend and began taking fosfomycin before anyone from my
office could reach out to her (she had some left over from a previous infection
with klebsiella).
In theory her current bug is sensitive to penicillin as well as cephalosporins
(and fosfomycin). Would you have her switch to a 1 gen cephalosporin or would
you have her finish the course of fosfomycin since she already began it? Thanks

SHOULD PROSPECTIVE PARENTS NEED A LICENCE TO HAVE CHILDREN?
I find it fascinating the hoops that couples who are looking to adopt a kid are
forced to jump through before being allowed to qualify for the possibility of
adoption. I understand the inscentive to verifying that the kid is going to a
good home. What really makes me scratch my head though is the contrast of how
"easy" (barring pregnancy or fertility complications ect) it is from a legal
perspective to reproduce. All you really need is two fertile consenting adults.
I recognize the issues with the following question but I would very much like to
hear what my fellow doctors on this platform have to say on the subject. Do you
think people should need some kind of licence to have a baby? To clarify,
enforcment seems to me almost impossible but for the sake of the question, lets
assume that enforcuing such a law would be possible and moral.

WORKPLACE MISTREATMENT FOUND TO BE COMMON AMONG ALL PHYSICIANS
Research, published in JAMA Network Open, found mistreatment was common among
all physicians, but there were disparities in mistreatment by gender and race.
Women were twice more likely to report mistreatment than men. This study also
showed statistically significant differences in mistreatment by race.
While prior research has found medical students and residents experience
frequent mistreatment, there has been a lack of corresponding data on
mistreatment of practicing and attending physicians. According to the survey,
the most common form of mistreatment, reported by about 17% of physicians and
representing over 70% of all mistreatment events, was mistreatment by patients
and visitors, followed by mistreatment by other physicians. The most frequent
forms of mistreatment were verbal mistreatment reported by 21.5%, sexual
harassment by 5.4%, and physical intimidation or abuse by 5.2%.
The survey found gender disparities in the experience of mistreatment, with
women twice more likely to report mistreatment (31%) than men (15%), more likely
to experience any form of mistreatment, and more likely to experience sexual
harassment and verbal mistreatment. Previous studies have also found higher
rates of occupational distress among female physicians, which have been
attributed to imbalances in domestic responsibilities and to differences in the
work environment. The survey also found that the prevalence of mistreatment

Aura-t-on un jour trop de médecins en France ? La question peut sembler
saugrenue aujourd’hui, mais elle a pourtant bel et bien été soulevée par les
doyens de plusieurs facultés de médecine à l’occasion d’un colloque organisé à
l’Académie de médecine de Paris.
Une situation de « pléthore médicale »
« Compte tenu du nombre de jeunes qu’on forme en médecine [environ 10 500 par an
à l’heure actuelle], je pense qu’on n’est pas à l’abri de se retrouver un jour
dans une situation de pléthore médicale », a averti Patrice Diot, doyen
honoraire de la faculté de médecine de Tours. « On fait du yo-yo en

35 YEAR OLD MAN WITH CHRONIC FULLNESS IN HIS LEFT EAR
I've been seeing this 35-year-old man for several years. He's been complaining
of fullness in his right ear for about three years now, ever since he had COVID.
Initially, I thought it was related to the infection and assured him the
sensation would go away, but it hasn't. We've tried antihistamines, ibuprofen,
and even steroid nasal sprays. Sometimes he experiences mild relief, but
recently, due to increased work travel, the sensation worsens during flights and
is starting to interfere with his daily functioning. He consulted with his ENT,
who recommended a balloon dilation eustachian tuboplasty. I hadn’t heard of this
procedure before, but apparently, they insert a balloon through the nose into
the eustachian tube and inflate it, which dilates the tube and improves middle
ear ventilation. Has anyone heard of this procedure? Do you recommend it? Thanks

FOR THE FIRST TIME IN A DECADE, STROKE PREVENTION GUIDELINES GET AN UPDATE
New guidelines on stroke prevention published Monday in the journal Stroke. The
guidelines urge health care professionals to screen people for risk factors such
as high blood pressure, elevated cholesterol, high blood sugar and obesity. They
also recommend raising public awareness about the benefits of healthy lifestyle
changes such as quitting smoking, increasing physical activity, improving
dietary habits and getting better sleep. "This guideline is important because
new discoveries have been made since the last update 10 years ago," Dr. Cheryl
D. Bushnell, who led the panel of experts that wrote the new guidelines, said in
a news release.
The guidelines also urge health care professionals to screen patients for
sedentary behavior, a confirmed risk factor for stroke, and to counsel them to
engage in regular physical activity. Federal physical activity guidelines
recommend that adults get at least 150 minutes a week of moderate-intensity
aerobic activity or 75 minutes a week of vigorous aerobic activity, or a
combination of both, preferably spread throughout the week.
Women should be screened for sex- and gender-specific factors that can increase
their stroke risk, the guidelines recommend. Such factors include the use of
oral contraceptives and pregnancy complications such as high blood pressure or
premature birth. Treatment of elevated blood pressure during pregnancy and
within six weeks of delivery is recommended to reduce the risk of intracerebral
hemorrhage. Other risk factors include endometriosis, premature ovarian failure

CHANGES IN AUDITORY PERFORMANCE FOLLOWING A VIRTUAL REALITY MUSIC CONCERT
This study aimed to assess the risk of hearing damage, both at threshold and
suprathreshold levels, from the rising use of virtual reality (VR) headsets for
music concert experiences. Recreational noise exposure, particularly from
in-person concerts and festivals, already poses a significant hearing hazard for
young adults, often exceeding safe daily limits set by the National Institute
for Occupational Safety and Health. Although much research exists on hearing
loss from personal music players, there has been no investigation into the
potential hearing risks from VR headsets, a relatively new technology in the
consumer market.
What are your thoughts on this issue? Is the responsibility on tech companies to
produce prodcuts that are safer for ears or is it on the consumer to keep the
volume at an appropriate level?

EIGHT YEAR OLD BOY WITH HYPERPHAGIA AND OBESITY
An eight year old boy is reffered to an endocinology clinic for obesity. He has
5 other siblings who are all normal and healthy. His parents describe him as a
good eater and a little bit slower than his siblings. They mention his language
has been developiong slowly and as such they are working with a speech
therapist.

NEWBORN WITH MLD - PARENTS HOPING FOR GENE THERAPY
My close friend just had his new baby diagnosed with MLD. Fortunatly they told
him that his kid is eligble for Libmeldy. I am not a hematologist but this new
gene therapy seems to be a game changer from the litte information I managed to
find online. The patients parents are understandably extreamly concerned and I
told them I would try to get them some more clarity but since this is quite a
new treatment I have been struggling to find helpful information. I wanted to
croudsource here to see if anyone has any first hand experience with this drug.
What can parents expect for their kids during treatment? Are there any support
groups for families going through this? What are the insurance implications and
are there any foundations to help pepole deal with the exorbinant cost of this
lifesaving drug?
Thanks in advance for any and all insights - the family is incredibly greatful.

99 YEAR OLD WITH DYSPHAGIA
I have a 99 year old patient who is in relativly good health considering his
age. He lives with his 91 year old wife in an assisted living facility but they
are mobile and otherwise independant. He has recently been complaining about
dysphagia and upon workup, it was found that he has a large benign growth around
his esophagus that extends all the way into the chest. Given the nature of the
tumor and his advanced age, head and neck surgery is reluctant to operate,
however this has had a significant effect on his life. He loves to eat and has
been incredibly frustrated with his recent dificulty swallowing. He has even
lost some weight due to his struggle.
Does anyone know of any minimally invasive procedures or swallowing exercises
that could help my patient?

EFFECT OF UPCOMING US ELECTIONS ON REPRODUCTIVE RIGHTS
As a healthcare provider I have been struggling with the issue of reproductive
rights and their relevance in the upcoming US presidential election. On the one
hand, my job is to treat patietns and care for them - not dictate policy. On the
other hand, if policy interferes with my ability to care for my patients in the
best way possible, should that not influence how I vote? My question is if you
think healthcare policy is something doctors should be outspoken about and
should lobby for? Is this something that should be left up to lawmakers? Thanks
