NEIGHBOR TO NEIGHBOR

ASSISTING COMMUNITY ELDERS TO REMAIN SAFELY INDEPENDENT AT HOME

Our Living at Home Block Nurse Program was born out of concern community
residents had regarding the frailty of elders living amongst us. The program was
incorporated in 2002 although the idea began in 2001.

Our service area began for those 65yrs. and older in the city of Frazee and out
in a 5 miles radius around the city. We have a MN tax exempt status. In 7yrs. it
has expanded to include two cities, Frazee & Vergas plus the townships of
Burlington, Silver Leaf, Gorman, Candor, Hobart, and Dora.

We now serve an average of 100 elders monthy within our service area in an
effort to enhance their daily lives, guard their safety...
