The Bankruptcy Court approved proof of claim forms, procedures, and deadlines in this case on June 7, 2023 [Docket No. 107]. Pursuant to the Bankruptcy Court order, Customer Proof of Claim forms must be submitted electronically on this website or by hard copy mail prior to August 31, 2023 at midnight Eastern Time (the "General Bar Date"). Please Note: If you are a customer filing a proof of claim for balances of cryptocurrency or fiat associated with your account with the Debtors, your claim will be either against Bittrex, Inc. (U.S. customers) or Bittrex Malta Ltd. (non-U.S. customers). You do not need to file a claim against each of the Debtors. Privacy of Information Collected Customers Only: Customer Proof of Claim Form requires providing certain personal information serving to verify customer identity and to comply with Know Your Customer government regulations, including the Bank Secrecy Act, other anti-money laundering laws, and guidance promulgated by the Financial Crimes Enforcement Network. Such personal information includes social security number or tax identification number, full date of birth, name, address, driver’s license number or passport number, Username Email and Account ID. The Debtors and Omni Agent Solutions will make every effort to maintain confidentiality of customer information provided as part of filing a Customer Proof of Claim Form. Court rules require redacting from publicly available court filings confidential information such as social security numbers or tax identification numbers, complete birth date, the name of a minor, or a financial account number. Additionally, on June 8, 2023, the Bankruptcy Court entered an order authorizing the Debtors to omit from public court filings confidential customer information, such as customers’ names, physical addresses and email addresses, any other contact information [Docket No. 116]. As a security measure, the Customer Proofs of Claims will be uploaded to the claims website managed by Omni using secured encrypted transfer, and will be stored online in an encrypted form. Customer proof of claim forms will not be available for public viewing, as are regular creditor proof of claim forms. The information requested on the proof of claim form is being collected for the purposes of facilitating the debtors’ bankruptcy cases under the U.S. Bankruptcy Code, facilitating customer withdrawals, and processing any claim you may have against the debtors. Creditors who are not customers: Your proof of claim form must not contain medical records, complete social security numbers or tax identification numbers (only provide the last four digits), a complete birth date (only provide the year), the name of a minor (only provide the minor's initials) or a financial account number (only provide the last four digits of such financial account). PLEASE REVIEW YOUR PROOF OF CLAIM AND SUPPORTING DOCUMENTS AND REDACT ACCORDINGLY PRIOR TO UPLOADING THEM.
