PCMH RECOGNITION ANNUAL REPORTING: CONTINUED SUCCESS

Join us for a live webinar designed to share the upcoming Annual Reporting
Requirements for reporting year 2023. This webinar will help PCMH practices
understand the larger goals of Annual Reporting and prepare for annual
submission under the Patient-Centered Medical Home program.

Faculty will highlight ways to demonstrate sustained recognition status through
maintenance and continuous quality improvement efforts using data that a
practice is already collecting internally and for other sources (e.g., state
initiatives). Timelines for deliverables and performance expectations will be
key discussion items.


TARGET AUDIENCE

This webinar is geared towards individuals with one year of experience, or more;
collecting, reporting, and acting upon annual reporting data; or those who need
a refresher. This program is designed for individuals in the practice that work
with patients, and with PCMH recognition.


LEARNING OBJECTIVES

At the conclusion of this course, participants will be able to:

 * Outline the Annual Reporting requirements for PCMH recognition.
 * Describe the Annual Reporting process; including timelines, submission and
   audit.
 * Interpret Annual Reporting requirements for reporting year 2023.
