and self-sufficiency for vulnerable individuals by providing them with access to
specialized care and housing. Thanks to our solid link to a vast array of
community resources and a dedicated staff endowed with a rare combination of

HOUSING STABILIZATION SERVICES

Housing Stabilization Services (HSS) is a new Home and Community based Service
(HCBS) that supports people with disabilities in finding housing; setting a
basic living budget; creating natural community supports by connecting with
landlords and neighbors; and assisting people with understanding the rules and
responsibilities of a lease.

People in different situations are eligible. The program could be a good fit for
you if you’re already living somewhere that just isn’t working for you, if
you’re currently homeless, or if you’re living with a family member or you are
in an institution like hospital, Nursing Home, or intermediate care facility
(ICF). It also involves creating a housing focused person-centered plan based on
your needs, preferences, and informed choice.

HOUSING CONSULTATION

At RelianceWell our Housing Consultation provides help to a person who doesn’t
have MA case management develop a person-centered plan that addresses their
needs, wants, and goals for living in the community.


HOUSING TRANSITION

At RelianceWell, housing transition services are provided to help a person plan
for, find, and move into housing. We will provide all eligible members a housing
consultant that will work with you every step of the way.


HOUSING SUSTAINING

At RelianceWell, we understand that maintaining a stable housing situation once
you have moved in is just as important as choosing the ideal residence....
