CERTIFICATE OF CONDUCT AND NON-CRIMINAL FINGERPRINT PACKAGE

All appointments for fingerprinting services must be scheduled online. Walk-ins

     color, watch item, etc.) and date of birth ALONG THE a valid U.S. driver’s
     license with photo or Department Motor Vehicle pic registration card
     (issued in USA) or IDNYC (New York City identification card). 

Non-Criminal Fingerprints may be required for certain permits or licenses
