COMPLETING THE DS-2032 STATEMENT OF REGISTRATION FORM

General Notes

 * A signed copy of the DS-2032 must be submitted in DECCS
 * Company entities owned or otherwise controlled (e.g., subsidiaries and
   affiliates) may not register separately.
 * Only electronic payment will be accepted for registration fees.
 * Organizational chart must be attached and show all layers of the organization
   through the ultimate parent (U.S. or Foreign) identified in block 11. In
   addition, the organizational chart should include the subsidiaries and
   affiliates identified in blocks 9 and 10.
   
   * If your registration does not include any subsidiaries or affiliates
     identified in blocks 9 or 10 or ultimate parent identified in block 11 then
     organization chart is not required.
 * Changes in certain information contained in this application must be reported
   promptly in accordance with 22 CFR 122.4 and 22 CFR 129.8.
 * All relevant documentation must be provided with the DS-2032 form at the time
   of submission.
