It also provides tools to facilitate knowledge sharing and agile project management. Reminder: This is an open platform that does not intend for users to communicate sensitive or confidential information such as protected health information and personal identifiable information. MADie Issue Tracker MADie -------------------------------------------------------------------------------- The MADiE Issue Tracker provides support to the measure authoring and testing user community. The Measure Authoring Development Integrated Environment (MADiE) allows users to develop and test measures in an integrated workspace. MADiE supports QI-Core profile informed authoring and testing of FHIR measures as well as QDM v5.6. This tracker captures questions, feature requests and issues related to these tools. CMS Hybrid Measures CHM -------------------------------------------------------------------------------- The Hybrid Measures Issue Tracker is used to track issues related to hybrid measures used in CMS quality reporting programs including technical questions on implementation (e.g., the specifications, logic, value sets, QRDA). To submit a question regarding measure implementation and methodology (e.g., previous measure testing and development, the cohort inclusions, measure exclusions, approach to risk adjustment, assessment of the outcome, and the planned readmission algorithm), please follow the below instructions: (1) Access the Quality Q and A tool at https://cmsqualitysupport.servicenowservices.com/qnet_qa (2) Select “IQR – Inpatient Quality Reporting” in the Program field (3) Select “Hybrid Measures in the Topic” field. Reminders: The Hybrid Hospital-Wide Readmission measure is NOT an electronic clinical quality measure (eCQM). Comments on eCQMs under development PCQM -------------------------------------------------------------------------------- The eCQMs under development (PCQM) project is used to post measure specifications and to secure comments and feedback on electronic clinical quality measures (eCQMs) in development. CQL Issue Tracker CQLIT -------------------------------------------------------------------------------- The Clinical Quality Language (CQL) Issue Tracker provides a public place for the standards community to submit technical questions and issues related to the development and implementation of the CQL standard. CYPRESS Issue Tracker CYPRESS -------------------------------------------------------------------------------- The CYPRESS Issue Tracker tracks issues related to certification testing tools used for the clinical quality measures used in CMS quality reporting programs. Issues can include questions on the test cases and implementation discrepancies. eCQM Issue Tracker CQM -------------------------------------------------------------------------------- The Electronic Clinical Quality Measure (eCQM) Issue Tracker is used to track issues related to electronic clinical quality measures (eCQMs) used in HHS quality reporting programs including questions on implementation e.g., the specifications, logic, code sets, measure intent. eCQM Known Issues EKI -------------------------------------------------------------------------------- The Electronic Clinical Quality Measure (eCQM) Known Issues dashboard provides implementation information for eCQMs with known technical issues for which a solution is under development but not yet available in a published eCQM specification. This includes discrepancies between eCQM narrative and logic, value sets, and/or technical, standard, or logic-related issues for eligible hospital/critical access hospital (EH/CAH) and eligible provider/eligible clinician (EP/EC) eCQMs. QDM Issue Tracker QDM -------------------------------------------------------------------------------- The Quality Data Model (QDM) provides a structure for describing clinical concepts contained within quality measures in a standardized format, allowing individuals who monitor clinical performance and outcomes to communicate information concisely and consistently. The QDM Issue Tracker provides a public place for stakeholders to submit technical questions and issues related to the development and implementation of QDM. Stakeholders should submit questions/issues/comments to the extent that they: * Are directly impacted by the existing model and/or available guidance * Require additional guidance on using the model * Have feasibility concerns about the implementation of the model * May result in changes and/or additional guidance to the model Questions regarding specific program measure content, measure logic and or feasibility, measure implementation, measure reporting and EHR software testing (Cypress) should be addressed to the respective Jira issue tracker. QRDA Issue Tracker QRDA -------------------------------------------------------------------------------- The Quality Reporting Document Architecuture (QRDA) Issue Tracker is used to collect technical questions and issues identified during implementation and reporting with the QRDA standard. QRDA Known Issue Tracker QKI -------------------------------------------------------------------------------- The Quality Reporting Document Architecture (QRDA) Known Issues dashboard provides implementation information for QRDA Implementation Guides (IGs) or supporting documents with known technical issues for which a solution is under development but may not be published. This includes issues such as: discrepancies between conformance assertions and message text, non-critical conformance assertion errors and typographical errors. Measures Specification Sheets MSS -------------------------------------------------------------------------------- The Measure Specification Sheets (MSS) Issue Tracker is used for the public to review and provide comment on the technical specifications supporting the proposed measures related to the Insights Condition and Maintenance of Certification requirements.
