Get some things at CVS. Remember to keep the poll code that is on your receipt.

Follow the link to https://www.cvshealthsurvey.com. Choose either English or
Spanish as your language.

Enter the four-digit number that’s written on the back of your receipt.

You’ll be asked to enter the 17-digit Survey ID at the bottom of the ticket
after you choose your language. Pick up the red “Next” button when you’re ready.

Finish the survey by being honest with all of your answers. Remember the last
time you dealt with CVS and give them your opinion. There are questions in the
survey about the service, the staff, the cleanliness, the quality of the goods,
and other things.

Type in your information here. At the end of the poll, you must give your
contact information in order to enter the sweepstakes. You can then join the
contest and try to win the $1,000 card.

Minute Clinics, where people can get treatment for things like colds and
