history, race, prior stroke, history of diabetes, cardiac and renal disease,
among others.


HTN-STROKE TEACHING 320

Instructed patient about the damage that high blood pressure can cause to the
vessels walls. Fat and cholesterol collect in the damage spots forming a plaque.
Blood cells stick to the plaque, forming a mass called a clot. A clot can block
blood flow in the vessel.




INFECTION TEACHING 325

Instructed patient on how an infection may cause symptoms or not. This presence
or absence of symptoms is caused by the microorganism's injury to the body's
cell or by the body response to the invasion.


WOUND CARE TEACHING 550

Patient was instructed on the optimization of wound environment. Adequate
nutrition and hydration, remove nonviable tissue, maintain moisture balance,
protect the wound and peri-wound skin, eliminate or minimize pain, cleanse,
prevent and manage infection, control odor.


HYPERGLYCEMIA TEACHING 769

Patient was instructed on hyperglycemia. It is important to know that glucose
levels vary before and after meals, and at various times of the day. Sustained
high levels of blood sugar cause damage to the blood vessels and to the organs
they supply, leading to the complications of diabetes.




INTERNAL FIXATOR TEACHING 1439

The patient was instructed in fixator devices internal fixator positions a risk
for refracture and analysis preventive actions, care in support walking, and no
weight manner in anticipation of well-ordered by the doctor. The patient was
advised that some nails or large plates may be removed in a year.


NON-HODGKIN'S LYMPHOMA TEACHING 1477

The patient was instructed in lymphoma malignant in the importance of evading
wound and pain which can cause hurting and bleeding. The patient was advised to
evading large multitudes and persons supposed of having an active infection. The
patient was recommended to follow the chemotherapy routine.


ABNORMAL GLUCOSE TEACHING 1910

SN instructed patient / caregiver on abnormal glucose also known as impaired
glucose tolerance which is a term that refers to people who have blood glucose
level that are higher than normal, but are not high enough to be classified as
diabetes. SN instructed patient / caregiver on symptoms of abnormal glucose such
as increased thirst, frequent urination, fatigue, blurred vision. SN instructed
patient / caregiver on risk factors such as weight, waist size, inactivity, age,
family history. SN instructed patient / caregiver on healthy lifestyle choices
that can help prevent diabetes such as eating healthy foods, getting more
physical activity, losing excess pounds.


LISINOPRIL TEACHING 1933

SN instructed that the lisinopril is a drug of the angiotensin-converting enzyme
inhibitor class used primarily in treatment of hypertension. If any of the
following side effects occur please report to physician / nurse : Itching or
hives, swelling in your face or hands, swelling or tingling in your mouth or
throat, chest tightness, trouble breathing, blistering, peeling, red skin rash,
confusion, weakness, uneven heartbeat, trouble breathing, numbness or tingling
in your hands, feet, or lips, dark urine or pale stools.
