Referring patients is simple and secure!

Our goal at Edwards Health Care Services (EHCS) is to empower patients to take
control of their life by better managing their diabetes with the guidance of a
skilled healthcare provider or educator, like yourself!

To better serve you and your patients, below are the different ways you can
refer a patient to EHCS as well as necessary healthcare provider and patient
forms. If you’re interested in learning more about the products we carry and the

Ready to refer a patient? 

There are multiple ways to send your referrals; pick the option that is best for
you! Please include patient demographics, CMN, chart notes, and labs for pumps.

Pharmacy Benefits

At times, we understand that EHCS may not be able to process your patients
request through medical benefits. That’s why we are excited to introduce our
sister company, GEM Edwards Pharmacy. GEM Edwards Pharmacy is our accredited
mail-order pharmacy that provides great customer service, medications, and
diabetes supplies shipped direct to your patients home. Click here to refer
