IDENTITY SCREENING $13.75

A check of personal identifying information provided on an authorization &
release form. Based on the applicants supplied social security number, this
screening will reveal name and address information associated with the number.


CIVIL RECORD SEARCH (PER STATE) $21.00

A search of civil records regarding all judgments, Iiens, and civil suits filed
against an applicant within the past seven years.


EDUCATION VERIFICATION (PER SCHOOL) $15.00

Verification of an applicant's highest or most applicable level of schooling,
based on the information provided on an employment application and/or resume, in
order to ensure the applicant is representing himself or herself truthfully.


PROFESSIONAL REFERENCES (PER REFERENCE) $16.75

Interviews with an applicant's professional references to learn about an
applicant's characteristics l'rom those with whom the applicant has worked.


BANKRUPTCY RECORD SEARCH (PER STATE) $15.00

A bankruptcy record search will show all bankruptcies that were filed,
dismissed, or discharged regarding an applicant within the past ten years.


GOVERNMENT RECORD SEARCH $15.50

A search of the Specially Designated Nationals and Blocked Persons List
maintained by the U.S. Department of the Treasury, Office of Foreign Assets
Control, and the Denied Persons List maintained by the U.S. Department of
Commerce, Bureau of Industry and Security.

NATIONAL CRIMINAL RECORD / SEX OFFENDER REGISTRY SEARCH $17.50

An online check ofavailable state and county criminal record repositories;
prison, parole, and release files from state Departments of Corrections;
Administrative Offices of Courts; and other state agencies. The information
available varies from state to state. The National Sex Offender Registry
contains information on sex offenders from all 50 states, plus the District of
Columbia. The offense levels listed vary by state.


PREVIOUS EMPLOYMENT VERIFICATION (PER EMPLOYMENT) $16.00

Verification of an applicant's previous employment history as listed on the
employment application and/or resume, going back seven years. This includes
contact with a record-keeping source, as well as with a supervisory source, if
one is provided on the employment application.


PROFESSIONAL LICENSE VERIFICATION (PER LICENSE) $15.50

Verification listed on the employment application and/or resume.


MOTOR VEHICLE DRIVING RECORD (MVR) SEARCH $23.00

Records are obtained from state government agencies, such as the Department of
Motor Vehicles or the Department of Public Safety. Records will list an
applicanfs driver's license status, as well as any history of violations,
suspensions/ revocations, and accidents.


MASSACHUSETTS CRIMINAL RECORD INFORMATION (CORI) SEARCH $38.00

A Massachusetts Criminal Offender Record information (CORI) Search through the
Commonwealth of Massachusetts Department of Criminal Justice Information
Services (DCJIS). Clients must first register with the iCORI system. The level
ofaccess for each client is determined by the DCJIS.
