Cerebral is a mental health subscription that provides clients with ongoing,
comprehensive access to online care and medication management for a monthly
rate. Conditions that Cerebral treats include:

HOW DOES CEREBRAL WORK?

Cerebral offers long-term online care and medication management for a wide range
of mental health conditions.

You start by simply filling out a free emotional assessment: Just a few
questions to help us understand your symptoms.

From there, you can choose to subscribe to one of several different subscription
options depending on the condition(s) you are seeking treatment for (some are
only available in certain states).

Cerebral Mental Health Plans

Medication + Care Counseling plan

 * Meet with your prescribing provider: Your prescriber will determine your
   treatment plan and prescribe medication as appropriate. They support you
   throughout your mental health journey and are available for follow-up
   appointments to track progress, answer questions, and adjust your treatment
   plan as needed.
 * Get your medication delivered: If prescribed, we’ll ship your medication to
   your door each month.
 * Connect with your care counselor: Your care counselor provides behavioral
   health tools, strategies, and education during monthly video or phone
   sessions. You can also chat with your care counselor at any time throughout
   the month. In-network insurance clients do not have access to a care
   counselor.

Medication + Therapy plan

 * Meet with your prescribing provider: Your prescriber will determine your
   treatment plan and prescribe medication as appropriate. They support you
   throughout your mental health journey and are available for follow-up
   appointments to track progress, answer questions, and adjust your treatment
   plan as needed.
 * Get your medication mailed monthly: If prescribed, we’ll ship your medication
   to your door each month.
 * Engage with your therapist: Your licensed therapist will leverage
   evidence-based practices to treat mental illness and help you develop
   emotional and cognitive coping skills through up to 4 video or phone visits
   per month. Once you subscribe, you can choose which therapist is right for
   you.

   coach is right for you. 
 * In-network insurance clients do not have access to the Coaching plan.

Cerebral Opioid Use Treatment Plan

For the Medication-Assisted Treatment (MAT) plan

 * Meet regularly with a specialized prescriber to develop a customized
   treatment plan
 * Your prescriber will determine if Suboxone is the right medication for you.
   If prescribed, your medication will be available for local pharmacy pick-up
 * Address your opioid use and any additional health concerns under this plan
   through prescription medications (when applicable) and counseling 


WHAT IS CEREBRAL’S MISSION?

Our mission is to democratize access to high-quality mental health care for
all. 

Mental health issues affect roughly 50% of the general population, but factors
like cost, convenience, and fear of judgment often prevent people from accessing
quality mental health treatment.

At Cerebral, the care we provide is covered by a monthly subscription. Cerebral
also takes a one-stop shop approach to mental health care: we provide our
clients with both evidence-based therapeutic techniques and medication
management, as the combination of both types of care has been shown to have the
greatest benefits. Finally, Cerebral operates in a telemedicine model, meaning
you can schedule visits with your prescribing provider, therapist, or Care
Counselor when it’s most convenient for you.


DO I NEED A PRESCRIPTION TO USE CEREBRAL?

No, you don’t need a prescription to use Cerebral. Our prescribing providers are
well-trained and will work with you to determine the best treatment and
medication plan for you. 


IN WHICH STATES IS CEREBRAL AVAILABLE?

In order to prescribe medication, a prescribing provider has to be licensed in
the state where the client is located. Currently, we have prescribing providers
who are licensed to provide medication services in every U.S. state, including
Washington D.C.

We offer “Medication + Therapy” and “Therapy” plans in Alabama, Arizona,
Arkansas, California, Florida, Georgia, Hawaii, Illinois, Indiana, Kansas,
Kentucky, Louisiana, Maryland, Michigan, Minnesota, Mississippi, Missouri,
Nebraska, New Jersey, New York, North Carolina, Ohio, Oklahoma, Pennsylvania,
South Carolina, Tennessee, Texas, Virginia, and Wisconsin, where we have
licensed therapists. We are constantly working to expand!


DO I NEED TO PAY FOR SHIPPING?

No! Shipping is included in the cost of your subscription.


WHAT SHOULD I DO IF I'M IN A CRISIS?

If this is an emergency, call 911 OR 1-800-273-8255 – which is a 24-hour suicide
prevention line that can be called from anywhere in the U.S. If you don't feel
like you are in immediate danger, but need guidance regarding your medication,
please reach out to your therapist or Care Counselor by messaging them in your
Cerebral profile.

*If we ever have reason to believe you intend to hurt yourself or others, we may
need to share information with your emergency contact in line with established
safety protocols.

DOES CEREBRAL TREAT BIPOLAR DISORDER?

Our prescribing providers are well-equipped to treat bipolar disorder. We offer
medication, therapy, and counseling services and suggest you meet with one of
our prescribers to see if Cerebral could be a good fit for your bipolar
management.


IS THERE A CEREBRAL MOBILE APP?

There sure is - now you can access quality mental health care wherever you go!
You can download the Cerebral mobile app through your app store, and keep us
right in your pocket.


DOES CEREBRAL TREAT ALCOHOL DEPENDENCE?

Our prescribing providers are well-equipped to provide care for alcohol
dependence. We offer therapy, counseling services, and medication treatment.
Medications we may prescribe for alcohol use disorder include naltrexone,
disulfiram, and acamprosate.

Each of your care team members is trained to provide you treatment and support
to overcome your alcohol dependence. We suggest you meet with one of our
prescribers to see if our alcohol dependence treatment could be a good fit for
you.

We offer care for alcohol dependence in: Alabama, California, Colorado,
Connecticut, Delaware, Florida, Georgia, Hawaii, Idaho, Illinois, Indiana,
Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Mississippi,
Missouri, Montana, Nebraska, New Hampshire, New Jersey, New Mexico, New York,
North Carolina, Ohio, Oregon, Pennsylvania, Rhode Island, South Carolina,
Tennessee, Texas, Utah, Virginia and Wisconsin.


DOES CEREBRAL PROVIDE TREATMENT FOR OPIOID USE DISORDER?

Yes, Cerebral’s opioid use disorder treatment program is available in select
states. This is a Medication-Assisted Treatment program focused on
harm-reduction. You will meet with a prescriber on a regular basis and be
prescribed Suboxone to help manage cravings and reduce the risk of relapse.
Patients will need to take randomized drug tests to ensure adherence to the
program. Patients will also be offered monthly counseling sessions to help
develop healthy habits and coping mechanisms.

We offer treatment for opioid use in: Colorado, Florida and Washington.

IS MEDICATION INCLUDED IN THE SUBSCRIPTION COST?

While we include the cost of delivery as part of your Cerebral “Medication +
Care Counseling” or “Medication + Therapy” subscription, if the prescriber deems
it appropriate to prescribe medication, medication costs will be billed
separately. That way, if you have insurance, you can use your insurance to cover
eligible medications. 

For self-pay clients, most medications will cost $10 out-of-pocket, but the cost
may be higher for some medications.


CAN CEREBRAL HELP ME CHANGE/GET OFF MY MEDICATIONS?

Your Cerebral prescriber can help you adjust your medication plan if that is
something you are interested in doing. They can help you create a safe plan to
either switch medications or go off of them entirely. Your therapist or Care
Counselor will also support you during this transition with ongoing visits.


ARE ANTIDEPRESSANTS SAFE TO USE?

Yes! Antidepressants are a highly effective means of treating anxiety,
depression, and insomnia. If you and your prescriber decide medication is the
best course of action for you, they will help guide you in the safest and
easiest way to begin your medication management. 

We prescribe non-controlled medications, which means that the medications aren’t
classified as having euphoric or addictive properties. There are no cravings, no
hazardous behaviors, and no examples of prolonged addictive behavior associated
with these medications. However, like most antidepressants, a withdrawal effect
can be experienced when stopping or reducing these medications without
consulting with your prescriber first.


WHAT ARE AS-NEEDED MEDICATIONS AND HOW CAN I GET THEM?

As-needed medications are medications that do not need to be taken on a regular
basis, but that can help in times of acute need.  

The cost of these medications varies. You can contact your Cerebral prescriber,
your Care Counselor, or therapist to find out more details after your first
prescriber visit.


IF I HAVE ADHD, WILL CEREBRAL PRESCRIBE ADDERALL (AMPHETAMINE /
DEXTROAMPHETAMINE) OR CONCERTA (METHYLPHENIDATE)?

No, Adderall (amphetamine/dextroamphetamine) and Concerta (Methylphenidate) are
controlled substances and cannot be offered through Cerebral at this time. Our
prescribers, however, are able to treat ADHD with non-stimulant options such as
Wellbutrin (bupropion) or Strattera (atomoxetine).


IF I HAVE SEVERE GENERALIZED ANXIETY OR PANIC DISORDER, WILL CEREBRAL PRESCRIBE
XANAX (ALPRAZOLAM) OR KLONOPIN (CLONAZEPAM)?

No, Xanax (alprazolam) and Klonopin (Clonazepam) are controlled substances and
cannot be offered through Cerebral at this time. Our prescribers, however, are
able to treat panic disorder with non-controlled substances such as Prozac
(fluoxetine), Zoloft (sertraline) or Effexor (venlafaxine).

HOW SOON CAN I SEE A PRESCRIBING PROVIDER?

Once you sign up with Cerebral, you will be prompted to schedule your first
video or phone visit with your prescriber. We aim to keep wait times as short as
possible, typically less than 7 days.

If you need to see a prescriber sooner than your scheduled appointment, please
message your coordinator by logging in to getcerebral.com and using the secure
in-app messaging system. We can help determine if any earlier appointments are
available.


HOW DO I KNOW CEREBRAL USES REAL MEDICAL PRESCRIBERS AND PHARMACIES?

We are a physician-led company and have selected the most capable
board-certified psychiatrists, physicians, physician assistants, and nurse
practitioners to join our team.


I HAVE ALREADY BEEN DIAGNOSED WITH A MENTAL HEALTH CONDITION. CAN MY PRESCRIBER
HELP ME MANAGE THE CONDITION?

Yes, depending on your condition. Cerebral care teams can treat a variety of
conditions, including but not limited to: 

 * Anxiety
 * Depression
 * Insomnia
 * Postpartum depression
 * Menopausal depression
 * ADHD (in certain states)
 * Bipolar disorder (in certain states)
 * Trauma and PTSD (in certain states)
 * Alcohol dependence (in certain states)

You are encouraged to share any past care you have received for your mental
health with your Cerebral prescriber. They can use that to inform your treatment
and medication plans moving forward.


ARE CARE COUNSELORS THERAPISTS?

Our care counselors are well-trained in therapeutic techniques, such as
behavioral activation, motivational interviewing, and more. Some, but not all,
Cerebral care counselors are licensed therapists.


AM I REQUIRED TO HAVE A VIDEO VISIT WITH MY PRESCRIBER? 

In most cases, you will be given the option to select a video or phone call
preference for all visits! For clients whose insurance carriers are in-network
with Cerebral, the visit must be conducted via video.


HOW OFTEN WILL I MEET WITH MY THERAPIST?

Therapy sessions are scheduled week-to-week. If you cannot make your therapy
session, make sure to cancel it at least 24 hours in advance.


CAN CEREBRAL PROVIDERS DIAGNOSE AND PRESCRIBE MEDICATION FOR ADHD?

Yes to both. We offer non-stimulant treatment options for ADHD.  At this time,
our providers do not offer stimulant medications for ADHD.


WHAT'S THE DIFFERENCE BETWEEN A THERAPIST AND A COACH?

Cerebral coaches provide clients with a goal-focused, skill-building approach to
tackling any personal and/or professional issues that may be causing distress.
Clients may, for example, seek the support of a coach to help them improve their
interpersonal relationships, self-image, or to address any destructive habits
that may be hindering their success. 

You can think of coaching as a more intensive form of care counseling that
involves introducing tools, strategies, and education aimed at supporting a
client’s personal development. Much like counseling, coaching also typically
focuses externally to support clients through education and encouragement of
actionable tasks, but with an emphasis on achieving personal goals.

Visits with a coach are included in our Medication + Coaching and Coaching Plan,
both of which are available to self-pay clients in most states. In these plans,
you can meet with your coach up to 4 times a month, and each session is 30
minutes in length. You can also message your coach through your patient portal
at any time.

Please note that coaches are not therapists. They work alongside prescribers
under our Medication + Coaching Plan, or independently as part of our Coaching
plan. They do not diagnose or treat mental illness. Only our prescribers and
therapists diagnose and treat mental illness.
Please also note that coaching is currently only available to new subscribers in
certain states. You can check if Coaching is available to you by going to

available to you at sign-up, please ask about upgrading your Medication + Care

WHAT TOOLS AND TECHNIQUES DO CEREBRAL THERAPISTS USE, AND WHAT CREDENTIALS DO
THEY HAVE?

A therapist compassionately holds space for clients and helps them process and
integrate difficult emotions and experiences. The overall goal of therapy is to
reduce symptom severity and help the client develop healthier thinking patterns
so that they can thrive in all areas of life. 

Cerebral therapists use a variety of evidence-based approaches to support their
clients. All of these approaches are grounded in solid scientific evidence found
in rigorous clinical trials. These include Cognitive Behavioral Therapy (CBT),
Dialectical Behavior Therapy (DBT), Psychodynamic Psychotherapy, Eye Movement
Desensitization Reprocessing (EMDR), and more.

Our therapists all hold a doctoral or master's graduate degree (PsyD, PhD, MSW,
MSEd, etc) from an accredited institution. They all maintain or are in the
process of obtaining licenses to practice in one or more states. Some of our
therapists hold Associate level licenses, which means that they are working
toward independent licensure under the direction of a professional clinical
supervisor. Licenses include: Licensed Clinical Social Worker, Licensed Mental
Health Counselor, Licensed Professional Counselor, Licensed Clinical
Professional Counselor, and Licensed Marriage and Family Therapist. 


WHAT KINDS OF THERAPISTS WORK AT CEREBRAL?

Cerebral therapists are licensed professionals with training in a variety of
evidence-based practices to treat mental illness. They help clients develop
emotional and cognitive coping skills to help reduce the severity of their
symptoms. Our therapists are well-equipped to address more severe and ongoing
symptoms stemming from, for example, trauma and childhood abuse. They can also
address and treat substance dependence (alcohol, opioid use, etc). Therapy,
especially at the beginning of treatment, typically focuses internally to help
clients explore and process their emotions with the goal of using this knowledge
to put actionable strategies into place to target and treat specific mental
health symptoms.

Rather than just asking “what strategies can we use to target these symptoms?”
therapists also ask “why are these symptoms occurring in the first place?” As
such, clients interested in uncovering the root of their symptoms in addition to
treating them would benefit most from therapy.

Research suggests that treatment involving a therapist, coach, or  Care
Counselor, in tandem with medication, yields the best treatment results.

Cerebral therapists can work with clients individually without the use of
medication. However, if medication is prescribed to you, your therapist will
work with your prescriber to ensure that your treatment plan is working
effectively.

Visits with a therapist are included in our Medication + Therapy and Therapy
plans, which are available to clients in most states. In these plans, you can
meet with your therapist up to 4 times a month, and each session is 45 minutes
in length. You can also message your therapist through your patient portal at
any time.

HOW DO I KNOW IF THERAPY IS RIGHT FOR ME?

The ideal candidate for Cerebral therapy would be someone who:

 * Is interested in developing coping strategies to support recovery from a
   mental illness in addition to uncovering the root of how their symptoms may
   have developed in the first place.
 * Is experiencing severe, or ongoing symptoms of a diagnosed (or diagnosable)
   mental health condition(s)

Is looking for long-term treatment strategies to address and treat their mental
health symptoms


WHAT IS A CARE COUNSELOR?

Cerebral care counselors are mental health professionals who provide clients
with behavioral health tools, strategies, and education aimed at improving their
mental health symptoms. They also maintain an ongoing relationship with your
prescriber to ensure that your treatment plan is working effectively. Care
Counseling typically focuses externally to support clients through education and
encouragement of actionable tasks proven to improve symptoms of mental health.

Visits with a Care Counselor are included in our Medication + Care Counseling
Plan for self-pay clients. In these plans, you can meet with your Care Counselor
once a month, and each session is 30 minutes in length. You also have the option
to complete additional Care Counseling visits at an added cost. You can message
your Care Counselor through your patient portal at any time.

Please note that Care Counselors are not therapists. They work alongside
prescribers under our Medication + Care Counseling Plan. They do not diagnose or
treat mental illness. Only our prescribers and therapists diagnose and treat
mental illness.


WHAT TOOLS AND TECHNIQUES DO CEREBRAL CARE COUNSELORS USE TO SUPPORT CLIENTS,
AND WHAT CREDENTIALS DO THEY HAVE?

At the start of treatment, behavioral health techniques used by Cerebral Care
Counselors typically include mindfulness, meditation, breathing exercises, and
grounding exercises.
As your treatment progresses, skill-building exercises such as goal development,
motivational interviewing, cognitive-behavioral based coping strategies,
psychoeducation, and behavioral activation are implemented to help increase your
ability to manage your mental health.

The majority of our Care Counselors have a Master’s degree in Social Work,
Psychology, or Counseling. Additionally, all Care Counselors undergo extensive
professional development training to remain up-to-date on the latest research
and best practices used in the industry.


HOW DO I KNOW IF CARE COUNSELING IS RIGHT FOR ME?

The ideal candidate for Cerebral care counseling would be someone who:

 * Wants to learn coping strategies to address mild to moderate symptoms of a
   diagnosable mental health condition
 * Does not have a longstanding history of mental health symptoms stemming from
   more severe conditions, such as PTSD or a personality disorder
 * Is not experiencing any additional serious health complications, such as
   substance dependence or an eating disorder
 * Is looking for more short-term strategies to manage their mental health
   symptoms


WHAT IS A COACH?

Cerebral coaches provide clients with a goal-focused, skill-building approach to
tackling any personal and/or professional issues that may be causing distress.
Clients may, for example, seek the support of a coach to help them improve their
interpersonal relationships, self-image, or to address any destructive habits
that may be hindering their success. 

You can think of coaching as a more intensive form of care counseling that
involves introducing tools, strategies, and education aimed at supporting a
client’s personal development. Much like counseling, coaching also typically
focuses externally to support clients through education and encouragement of
actionable tasks, but with an emphasis on achieving personal goals.

Visits with a coach are included in our Medication + Coaching and Coaching Plan,
both of which are available to self-pay clients in most states. In these plans,
you can meet with your coach up to 4 times a month, and each session is 30
minutes in length. You can also message your coach through your patient portal
at any time.

Please note that coaches are not therapists. They work alongside prescribers
under our Medication + Coaching Plan, or independently as part of our Coaching
plan. They do not diagnose or treat mental illness. Only our prescribers and
therapists diagnose and treat mental illness.
Please also note that coaching is currently only available to new subscribers in
certain states. You can check if Coaching is available to you by going to
Cerebral.com and scrolling down to 'Our Membership Plans.' If coaching is not
available to you at sign-up, please ask about upgrading your Medication + Care
Counseling plan after you subscribe!


WHAT TOOLS AND TECHNIQUES DO CEREBRAL COACHES USE TO SUPPORT CLIENTS, AND WHAT
CREDENTIALS DO THEY HAVE?

Cerebral coaches implement a wide variety of evidence-based behavioral tools and
interventions to assist in the personal development of their clients. This
typically includes, goal-development, active listening, motivational
interviewing, generative brainstorming, acting as an accountability partner, and
providing instruction and challenges to help clients create and maintain
routines to achieve their goals.

The vast majority of Cerebral coaches have a Master’s degree in Social Work,
Psychology, or Counseling. Additionally, all coaches undergo extensive
professional development training to remain up-to-date on the latest research
and best practices used in the industry.


HOW DO I KNOW IF COACHING IS RIGHT FOR ME?

The ideal candidate for Cerebral coaching would be someone who:

 * Is growth-oriented rather than symptoms-focused in their approach to
   improving their mental health
 * Has one, or several personal and professional issues that they’d like to
   improve upon through the use of goal-setting and self-development strategies
 * Is dealing with mild to moderate mental health symptoms (such as anxiety or
   depression) as a direct result of feeling stuck in a particular area (ex: “I
   don’t know how to communicate with work colleagues,” or “I don’t feel good
   about my body image.”)
 * Does not have a longstanding history of mental health symptoms stemming from
   more severe conditions, such as PTSD or a personality disorder
 * Is not experiencing any additional serious health complications, such as
   substance dependence or an eating disorder
 * Is looking for more short-term strategies for identifying and implementing
   goals

CAN I USE INSURANCE WITH CEREBRAL?

Yes, but please note that FSA funds can only be applied to Cerebral plans if you
are paying for your subscription out of pocket. 

For most of our plans, we accept insurance for select major commercial health
plans, and we are constantly working to expand our in-network insurance
partnerships.

subscription reimbursement. Clients will need to attend at least one appointment
within the billing cycle for their insurer to review the claim.

Cerebral does not currently accept clients that are covered by Medicaid or
Medicare Part B.


HOW MUCH DOES IT COST? 

If you’re on the Medication + Care Counseling plan, your membership will start
at $99 per month. Prescriptions will be billed separately and are eligible for
insurance coverage. The subscription includes ongoing video or phone
appointments with your prescriber, unlimited care team messaging, the cost of
medication delivery, and if you do not have in-network insurance, monthly video
or phone visits with your Care Counselor. If your prescriber determines that it
is necessary to meet with you on a more frequent basis for treatment monitoring,
there may be an additional charge for the visit. 

If you’re on the Medication + Therapy plan, your membership will start at $325
per month. Similarly, your prescriptions will be billed separately and are
eligible for insurance coverage. This subscription includes everything in the
“Medication + Care Counseling” plan, plus weekly sessions with a licensed
therapist instead of monthly visits with a Care Counselor. If your prescriber
determines that it is necessary to meet with you on a more frequent basis for
treatment monitoring, there may be an additional charge for the visit. 

Cerebral’s Therapy plan starts at $259 per month. This subscription includes
weekly sessions with a licensed therapist. 

If Cerebral is in-network with your insurance plan, your monthly subscription
fee starts at just $30 per month under all of our plans. The monthly
subscription fee does not include the cost of any co-pays for visits with
providers or medication costs, which will vary depending on your insurance plan.
Please note that the monthly subscription fee and any co-pays are your
responsibility, not your health plan's, even if Cerebral is considered
in-network.

For all of our clients whose insurance is not yet considered in-network, we can
try to get you reimbursed for our services from your insurance carrier. Enter
your insurance information into your account, and we’ll work with you to submit
the claims to your insurer. 

Cerebral’s Nutrition-Only plan starts at $69 per month. This subscription
includes monthly sessions with your registered dietitian. 

Cerebral’s Medication-Assisted Treatment + Counseling plan for Opioid Use starts
at $195 per month. This subscription includes regular sessions with your
prescriber and monthly counselor sessions. Prescriptions will be billed
separately and are eligible for insurance coverage.

DO I NEED INSURANCE TO SIGN UP?

No! We will support your mental wellness regardless of your insurance access.


DOES CEREBRAL PROVIDE LETTERS OF CARE, FMLA, AND ESA DOCUMENTATION?

Your Cerebral prescribing provider can provide you with certain records and
documents, according to the prescriber’s discretion. 

Members can request documents regarding issues like disability, work leave,
Family and Medical Leave Act (FMLA), and emotional support animals (ESA).

The prescriber will decide whether or not to provide the document. It might be
the case that your level of interaction with the prescriber is too limited to
warrant them providing certain documents. However, we’re always happy to forward
document requests to your prescriber for their input. 

If we can’t provide the requested paperwork, we can always provide a copy of
your records, in conjunction with a letter of care, indicating that you’re using
Cerebral’s services.


HOW DOES INSURANCE WORK FOR MY MEDICATION?

Prescriptions are billed separately from your monthly subscription, giving you
the opportunity to use insurance for your medication! 

If you’re covered, you’ll pay for your medications according to your insurance
agreement. That means you may have copay or deductible costs. If you remain on a
self-pay plan, your medication will be billed according to a tiered pricing
system. Most medications will cost $10 out-of-pocket, but the cost may be higher
for some medications.


HOW DO I USE HSA/FSA TO COVER MY CEREBRAL SUBSCRIPTION?

There are two ways in which you may use HSA/FSA funds to cover your Cerebral
subscription:

1. If you have your HSA/FSA connected to a card, you may enter that card
information as your payment method on file.

2. If you need to submit a receipt for reimbursement, you may request an
itemized receipt by emailing us at support@getcerebral.com.


IS THERE A COMMITMENT?

No, Cerebral is a month-to-month subscription and can be canceled at any time.
However, for the most effective treatment, it is beneficial to continue with a
long-term treatment plan.


HOW DO I CANCEL MY SUBSCRIPTION?

You can cancel your subscription at any time by navigating to your membership
plan details section in the patient portal. 



1. Log in to your account via mobile app or web browser

2. Visit the "My Account" page by clicking or tapping the menu button at the top
right of your screen

3. Click or tap" View membership plan details" under your subscription plan

4. Click or tap "Cancel Subscription"

If you're unable to see these buttons on the mobile app, please ensure that you
have updated to the latest version of the Cerebral app.

It's important to note that your subscription will remain active until these
steps have been completed. After your subscription cancellation has been
processed, your treatment and medication plan will be suspended. However, you
will still have access to your medical records through your Cerebral patient
portal.

WHY DO I HAVE TO UPLOAD A PICTURE OF MY FACE OR MY GOVERNMENT-ISSUED PHOTO ID?

Your prescriber will need to confirm your identity and date of birth before you
can be treated via telemedicine.

We require an identifiable photo of your face. The photo needs to be taken in
real-time and needs to be a clear and accurate representation of you at the
point in time of your online visit (i.e. not old photos of you from years ago).

 * No sunglasses or other facial obstructions
 * No pictures of pictures
 * No filters

We also require an identifiable photo ID. This must include:

 * First Name
 * Last Name
 * Date of birth
 * Expiration date (for legal purposes your ID cannot be expired)
 * Unobstructed photo of your face
