 * Applicant or representative MM2H company must be present;

   company;
 * Original & copy passport participant (particular page, visa page and latest
   entry stamp);
 * Original & copy conditional approval letter; and
 * Form Imm.12  IM.12.pdf (available at website JIM) with photo (blue
   background) for participant.

2. APPLICATION GUIDELINES FOR ISSUING OF SOCIAL VISIT PASS FOR SPOUSE/CHILDREN
(UNTIL 34 YEARS OLD)

CLIENT CHARTER: 30 (THIRTY) WORKING DAYS

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/agency representative letter for MM2H
   company;

INCOME)

5. APPLICATION GUIDELINES FOR RENEWAL OF 5 YEARS SOCIAL VISIT PASS (FIXED
DEPOSIT)

  CLIENT CHARTER: 30 (THIRTY) WORKING DAYS

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Original & copy of applicant passport;
 * Form Imm.55  Imm.55.pdf (available at website JIM);
 * Latest account statement (fixed deposit account);
 * Current statement account;

6. APPLICATION GUIDELINES FOR TRANSFER OF ENDORSEMENT FROM OLD TO NEW PASSPORT
AND EXTENSION

  CLIENT CHARTER: 1 (ONE) WORKING DAYS

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of approval letter;
 * Form IM.12.pdf with photo (available at website JIM) with photo (blue
   background) for participant.
 * Form Imm.55.pdf (available at website JIM);
 * Original & copy of old & new passport for applicant & participant.
 * Applicant who is 21 years and above and holding MM2H dependant pass will not
   be given further extension.

7. APPLICATION GUIDELINES FOR PERMISSION TO STUDY

  CLIENT CHARTER: 14 (FOURTEEN) WORKING DAYS

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/agency representative letter for MM2H
   company;
 * Application must be below 18 years old (above 18 years old need to apply
   student pass );
 * Copy of approval letter;
 * Form student personal data  Student’s Personal Data.pdf (available at website
   JIM);
 * Original & copy of participant and applicant passport;
 * Letter of acceptance from school; and
 * Enrollment fee from school.

8. APPLICATION GUIDELINES FOR PERMISSION TO PART-TIME WORKING
 * Authorization Letter from the company (designation in company);
 * Cover Letter explaining company activities and job justification;
 * Offer Letter from the company (must mention duration of employment contract,
   duration of working hour and remuneration);
 * Latest print out Companies Commission of Malaysia Form 9,24 & 49;
 * Business licence from local authority;
 * Recommendation letter from related agency (which ever related);
   
   (i)  Education – Ministry of Education;
   
   (ii)  Health – Ministry of Health;
   
   (iii)  Manufacturing – Malaysia Industrial Development Authority;
   
   (iv)  Securities – Securities Commission;
   
   (v)  Banking. Financial and insurance – Central Bank Malaysia;

 * Advertisement in local newspaper/ jobstreet for the intake of local citizen;
 * Completed DP10 Form;
 * Participant’s resume;
 * Certified copy of participant’s passport; and
 * Personal income tax relief letter from Inland Revenue Board of Malaysia
   (LHDN)-salary above RM10,000.00.

9. APPLICATION GUIDELINES FOR NEW DOMESTIC HELPER

CLIENT CHARTER: 14 (FOURTEEN) WORKING DAYS

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/agency representative letter for MM2H
   company;
 * Form IM.12.pdf with photo (available at website JIM) with photo (blue
   background) for domestic helper’s;
 * Approval letter from Philippine Overseas Employment Administration (POEA) for
   domestic helper from Philippine only;
 * Passed medical check-up (from origin country of domestic helper);
 * Employment contract from origin country if the maid already working with
   prinsipal (non source worker);
 * Personal bond  Personal Bond.pdf (available at website JIM) stamped RM10.00.
 * Copy of domestic helper passport;
 * Copy of participant passport (particulars &MM2H visa pages);
 * Employment’s contract (stamped RM10.00 at duty stamp office) Employment
   Contract.pdf (available at website JIM-2 original copies);
 * Original & copy of SNP/tenancy agreement;
 * Original & copy of utilities bill (electric/water/astro); and
 * Original & copy of Marriage/birth certificate.

10. APPLICATION GUIDELINES FOR RENEWAL DOMESTIC HELPER

CLIENT CHARTER: 1 (ONE) DAY WORKING

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of approval letter;
 * Form Imm.55.pdf (available at website JIM);
 * FOMEMA approval (year 1/2/3 and odd years);
 * Original & Copy of applicant passport particulars &MM2H visa pages; and
 * Original & copy of domestic helper passport.

11. APPLICATION GUIDELINES FOR TERMINATION OF PASS

CLIENT CHARTER: 3 (THREE) WORKING DAYS

(i)  IN MALAYSIA

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/agency representative letter for MM2H
   company;
 * Authorize letter to withdraw fixed deposit from Immigration Department of
   Malaysia;
 * Original passport;
 * Original & copy of flight ticket; and
 * Copy of Conditional Approval Letter.

(ii)  TERMINATION PAS MM2H PARTICIPANTS IN ABROAD

 * Applicant must be present at nearest Malaysia Embassy/High Commision;
 * Intention letter for terminated pas MM2H;
 * Termination letter from MM2H Centre, Immigration Department;
 * Original passport (present at Malaysia Embassy/High Commision);
 * Copy of conditional approval letter; and
 * Latest exit endorsement/stamping cop from Malaysia

13. CHECKLIST FOR CHANGE OF PRINSICAL

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/agency representative letter for MM2H
   company;
 * Copy of Prinsipal’s Passport (Particular and MM2H Visa Page);
 * Copy of Dependent’s Passport (Particular and MM2H Visa Page);
 * Copy of Principal’s Death Certificate;
 * Copy of Conditional Approval Letter;
 * Certificate of Good Conduct (from country of origin);
 * Copy of Fixed Deposit Certificate (if approved on Fixed Deposit);

GUIDELINES FOR WITHDRAWAL OF FIXED DEPOSIT

Application Guidelines for withdrawal of Fixed Deposit for the existing MM2H
participants in Malaysia.

CLIENT CHARTER: 3 (THREE) WORKING DAYS

1. RESIDENTIAL PURCHASE

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of Passport (Particular Page and visa MM2H);
 * Copy of Conditional Approval Letter;
 * Copy of Fixed Deposit Certificate;
 * Copy of sales & purchase agreement (all pages);
 * Copies of receipt of a house payment.

NOTES:

 * Participant may submit a copy of the sales and purchase agreement (SPA)
   without the receipt of a home payment provided that the SPA is validated
   (Certified True Copy) by the relevant authorities.
 * Participant is allowed to request partial withdraw of Residential Properties
   ONLY.
 * Duty stamp page and signature page MUST be certified.

2. EDUCATION

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of Passport (Particular Page and visa MM2H);
 * Copy of Conditional Approval Letter;
 * Copy of Fixed Deposit Certificate;
 * Copies enrollment fee from school.

3. MEDICAL EXPENSES

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy Of Passport Particular Page;
 * Copy Of MM2H Visa;
 * Copy Of Conditional Approval Letter;
 * Copy Of Fixed Deposit Certificate;
 * *Copy proof of payment for Medical Expenses (Receipt)

NOTES:

 * Participant may submit a receipt of payment for medicines purchased at
   Pharmacy or other related fee such as consultation, hospital admission as
   proof of medical expenses.

4. INTEREST

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of Passport (Particular Page and visa MM2H);
 * Copy of Conditional Approval Letter; and
 * Copy of Fixed Deposit Certificate.

5. TRANSFER OF BANK

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of Passport (Particular Page and visa MM2H);
 * Copy of Conditional Approval Letter; and
 * Copy of Fixed Deposit Certificate.

6. JOINT OR SEPARATE ACCOUNT/CHANGE TERM OR TENURE/ CHANGE BRANCH OF BANK/
CONVENTIONAL BANK

 * Applicant or representative MM2H company must be present;
 * Intention letter for individual/ agency representative letter for MM2H
   company;
 * Copy of Passport (Particular Page and visa MM2H);
 * Copy of Conditional Approval Letter; and
 * Copy of Fixed Deposit Certificate.

GENERAL NOTES

 * MM2H participant are allowed to withdraw partial of the required Fixed
   Deposit starting from the second year onwards (effective from the date of
   MM2H Social Visit Pass endorsement in the passport) for approved expenses
   related to residential purchase, children’s education expenses, medical
   expenses as per below:
   
   (a) RM50,000 for aged 50 years and above; and
   
   (b) RM150,000 for aged below 50 years.

 * However, MM2H participant are required to maintain the balanced of Fixed
   Deposit until they terminate their participation from Programme as per below:
   
   (a) RM 100,000 for aged 50 years and above; and
   
   (b) RM 150,000 for aged below 50 years.

 * Withdrawal of Fixed Deposit is on reimbursement basis and release of money
   will be made based on the amount requested.
 * All application for withdrawal of fixed deposit is no longer permitted to be
   submitted by e-mail.
   Any incomplete documents will not be processed. Complete documents will be
   process within three (3) working days.
