ONLINE SURVEY CONSENT FORM

Aloha! You are being asked to participate in a research study conducted by the
University of Hawai‘i Economic Research Organization (UHERO) and the Council for
Native Hawaiian Advancement (CNHA).


WHAT AM I BEING ASKED TO DO?

If you participate in this project, you will be asked to fill out a survey each
month.


TAKING PART IN THIS STUDY IS YOUR CHOICE.

Your participation in this project is completely voluntary. You may stop
participating at any time. If you stop being in the study, there will be no
penalty or loss to you.


WHY IS THIS STUDY BEING DONE?

This project will gather and disseminate real-time data on the housing and
employment conditions of Maui fire survivors. Currently, most of the existing
data is held by federal and state agencies. It is not publicly available and not
updated frequently. We want to make this data publicly accessible. This will
help to guide the recovery efforts of community organizations and the
government. It will also hold them accountable.


WHAT WILL HAPPEN IF I DECIDE TO TAKE PART IN THIS STUDY?

The survey will consist of multiple choice questions about your housing and
employment situation. It will take no more than 10 minutes to complete. We will
send you the survey every month. The first survey includes a few additional
questions and may take about 15 minutes to complete. The study will run for at
least 24 months. You can stop participating at any point. There is no obligation
to participate for the full study period and you can join again at any point.
You will receive a payment for your participation each month.


WHAT ARE THE RISKS AND BENEFITS OF TAKING PART IN THIS STUDY?

We believe there is little risk to you when participating in this research
project. You may become stressed or uncomfortable answering some of the survey
questions. If you do become stressed or uncomfortable, you can skip the question
or take a break. You can also stop taking the survey or you can withdraw from
the project altogether.

There will be no direct benefit to you for participating in this project. The
results of this project will contribute to guiding Maui recovery efforts.


COMPENSATION:

You will receive a $20 gift card for the initial survey and a $10 gift card for
every monthly survey after that.


RESULTS OF RESEARCH:

Each month, the overall results of the survey will be published online on a
publicly available interactive data dashboard at Maui Recovery Dashboard:
Housing and Jobs and in short blogs and research briefs at uhero.hawaii.edu.
Your individual answers to the survey questions will never be published.


CONFIDENTIALITY AND PRIVACY:

We will keep all study data on a secure server at the University of Hawai'i.
Only a small group of trained researchers from the University of Hawai‘i
Economic Research Organization and the Council for Native Hawaiian Advancement
will have access to the information. Other agencies that have legal permission
have the right to review research records. The University of Hawai'i Human
Studies Program has the right to review research records for this study.


FUTURE RESEARCH STUDIES:

After removal of identifiers, the research records may be used for future
research studies or distributed to another investigator for future research. We
will not seek further approval from you for these future studies.
