Allegheny Health Network makes it easier for patients to access a full range of
customized health care services — in the privacy and comfort of their own homes.
Through Allegheny Health Network’s Healthcare@Home program, we are helping many
patients to maintain their independence and continue the healing process at home
as long as possible.
ONE-CALL, ONE-STOP, HOME-BASED HEALTH CARE
Call 1-800-381-8080 to arrange for home health, hospice, palliative, and
infusion therapy services, as well as the delivery of medical equipment and
supplies. We will customize a program of care that meets your unique and
changing needs. Then we will arrange for services and equipment to be delivered
directly to your home.
EXPERIENCED, COMPASSIONATE STAFF READY TO PROVIDE SUPPORT
We know it can be difficult to deal with a sickness, injury, or accident. Our
goal is to make it as easy as possible for you to get the care you need,
whenever you need it. We have assembled a team of experienced, caring
professionals who will make you feel safe, supported, and well cared for in your
own home.
ACCESS TO OUR TEAM OF RENOWNED PHYSICIANS
There may come a time when you need more advanced medical care. That's when we
can refer you to one of Allegheny Health Network's 2,100 primary care physicians
or specialists. We will ensure that you receive a seamless continuum of care,
whether you are a patient in a hospital, at an outpatient care center, at a
doctor's office, or at home.
