Services to produce a Mainfreight Future Leaders Profile on Chiel Hesseling.

Instructions

The questionnaire form is divided into a number of separate pages. A progress

  Section One: Behaviours


In this section there are 48 behavioural statements that relate to the
Mainfreight Leadership Model.   Please read each statement carefully, then rate
the person being assessed in terms of how frequently they display the behaviour.
Where you are unable to form a judgement for any reason, then leave the item
blank. However, please try to leave as few items blank as possible.


 * You are given 11 choices ranging from never (0) through to always (10).

 * Using the in-between choices of seldom, sometimes, and often as guides,
   select any button in the range from 0-10 which best represents the extent to
   which the person being assessed actually engages in each behaviour.

 * In selecting each answer, be realistic about the extent to which the person
   being assessed actually engages in each behaviour, and answer in terms of how
   they typically behave, rather than how you think they should behave.

Note: If you rate the person a zero you are giving them feedback that they
'never' engage in a particular behaviour. If you think a zero is inappropriate
for any reason, for example the behaviour is not required for the role, then
please leave the item blank rather than rate a zero.



  Section Two: Written Feedback

This is a Written Feedback Section.   Please base your feedback on the work
performance of the person being assessed within the last six months.

Note: Please make sure you provide comments as recipients often find comments to
be of real value. Your comments will be made available verbatim to the person
being assessed so avoid comments that will compromise your anonymity. You will
not be identified by name unless you are the supervisor.
