MORTALITY DUE TO COEXISTING DIABETES, HEART FAILURE RISING IN OLDER AMERICAN
INDIAN ADULTS

Rates of mortality from coexisting heart failure (HF) and diabetes mellitus (DM)
have risen in older adults who are American Indian or Alaska Native over the
past 20 years, according to a study published in the International Journal of
Cardiology: Cardiovascular Risk and Prevention.

The researchers analyzed death certificates from the CDC WONDER database,
measuring rates of mortality caused by co-occurring HF and DM in adults aged 75
years and older between 1999 and 2020. They adjusted for year, gender, race, and
region when calculating age-adjusted mortality rates (AAMR) and annual percent
change (APC).

In total, 663,016 deaths were reported in patients with coexisting DM and HF.
Over the study period, the AAMR increased from 154.1 to 186.1 per 100,000
people. The authors noted a significant increase between 2018 and 2020, with an
APC of 11.3.

Older men had higher AAMRs than older women, at 185 versus 135.4 per 100,000
people. AAMRs were highest among patients who identified as non-Hispanic
American Indian or Alaska Native (214.4/100,000) and lowest among patients who
were Pacific Islander (104.1/100,000). However, patients who were Asian or
Pacific Islander also saw a significant increase between 2014 to 2020 (APC,
1.99). AAMRs were highest in the Midwest region and in individuals living in
non-metropolitan areas.

“Selecting an elderly cohort aged 75 and above as our subjects, we observed
elevated rates predominantly among [non-Hispanic] American Indians or Alaska
Natives, males, individuals residing in the Midwest region of the United States,
and those dwelling in non-urban areas,” the authors summarized. “These findings
underscore the pressing need for systemic reforms in healthcare practices and
protocols.”
