FISHER SERVICE AWARD





WELCOME TO THE 2023 FISHER SERVICE AWARD APPLICATION PORTAL!



All users must create a new user account. Even if you have applied in previous
years. Each award cycle, we erase all data resetting our application portal, so
your information will not be found. 

There are three parts to the application, which you can complete in any order
you choose.

You may return at any time to modify your application before the application
period closes on Thursday, March 23, 2023, at 9:00 PM EST. Once the closing date
has passed, no more modifications can be made — there are no exemptions or
exceptions to this deadline.

Remember, your organization must be a not-for-profit organization as defined in
Section 501(c) of the Internal Revenue Service Code.

DATA GOVERNANCE POLICY

Information collected from award applicants is intended only for the use of
Fisher House/Fisher Service Award to evaluate applicants and may contain
confidential and/or privileged material. Any review, re-transmission,
dissemination, or other use of this information by persons or entities other
than Fisher House/Fisher Service Award is prohibited.

DATA GOVERNANCE PROCEDURES

 * Only information necessary for evaluating applications and analyzing usage
   should be collected from applicants.
 * Review all information collected from applicants to ensure no unnecessary
   sensitive information is stored and maintained.
 * Password protect all databases and limit administrative access to Fisher
   Service Award and Confluent Design staff
 * Do not send exported data or databases to external parties (other than
   Confluent Design for troubleshooting bugs/glitches in the online system.)
   * When asked for information regarding Fisher Service Award, do not provide
     access to the database.
   * Fisher Service Award may only provide general statistics (i.e. number of
     applicants, number of awards, applicant categories, etc.) as it relates to
     the award program.
 * Do not export reports with applicant information or save a copy of the
   database on personal drives/computers.
