In addition to being a Fellow of the Royal College of Psychiatrists, Helen serves the organisation as a specialist advisor for workforce. She chairs the Royal College of Psychiatrists Workforce Committee and is involved in several programmes with the College related to leadership, workforce innovation, and transformation of services. Helen is also a Fellow of the Academy of Medical Educators and a Senior Fellow at the Faculty of Medical Leadership and Management. She is a member of Sciana's fifth cohort. Ahead of Cohort 5’s fourth residential meeting, Helen reflected on her Sciana journey. The opportunity to become a Salzburg Global Fellow and join Sciana: The Health Leaders Network had been on my radar for a while. Working as a clinician, academic, and manager and having benefitted from previous developmental opportunities, I was attracted to the idea of taking my leadership to the next level, collaborating with colleagues working in health across Europe. The Sciana Network is now in its sixth cohort, with each comprising 18 leaders from Germany, Switzerland, and the UK. Sciana, a portmanteau word from the Esperanto words 'scias' (to know) and 'sana' (healthy), is supported by the Health Foundation (UK), Careum (Switzerland), and Bosch Health Campus (Germany) in collaboration with Salzburg Global Seminar. Our cohort – Cohort 5 - met remotely over the two-year programme and several times at Schloss Leopoldskron in Salzburg, Austria. Schloss Leopoldskron is the historic home of Salzburg Global, a grand, moving, and history-rich building that makes a fabulous setting for learning and living together. Cohort 5 comprises community leaders, innovators, patient representatives, clinicians, academics, economists, public health experts, health managers, and others, representing diverse stakeholders coming from very different perspectives. At the start of our programme in 2023, we spent some time understanding more about our backgrounds, priorities, concerns and passions. We were exceptionally well-supported by Chairs and Advisors and eventually found ourselves coalescing around the themes of workforce, health literacy, and digital through the lens of One Health. One Health is a term I hadn’t heard of prior to the Fellowship, although it was familiar to others. It is an approach advocated by the World Health Organization utilising a transdisciplinary collaborative approach to look at the co-dependencies of animal, human, and planetary health. The aim is to harness a wide range of expertise and experience to design and implement novel health policies and actions to achieve better public health outcomes and address health threats in the animal-human-environment interface, which we have become all too familiar with from our experiences of the COVID-19 pandemic. It has been humbling and, at times, overwhelming to hear other leaders talk about their experiences of trying to enact change, facing struggles, and hearing of the challenges of finding ways forward to address these. There are, of course, different cultures, health systems, and political and economic drivers across the three countries, and potential solutions in one system will not necessarily work elsewhere. Using action learning approaches, we have tried to support one another, offering feedback and new insights into familiar problems. An overarching theme emerged of how to work with citizens and users of our health systems. We have spent time exploring power differentials and health inequalities, considering how to amplify health literacy and governance around One Health. It is clear to us the importance of the codesign principles, which will be necessary in leading any change with stakeholders. In one exercise, we played out an imaginary scenario from the future, asking what we would have done differently today had we known what our “imagined” future would look like 30 years from now. It was a powerful day. It exposed our deep-seated anxieties and uncertainties about the health of future generations – our children and grandchildren! What would the geopolitics look like in 30 years? Would democracy survive? How many other novel viruses would have emerged? What would the impact of climate disasters or mass migration be? And how would we see the challenges of securing health for humans, animals, and our planet in this imagined world? It was sobering but also enlightening to think about the type of leadership needed now to face some of these issues. Brave leadership was, of course, an overarching theme that we came back to again and again. In thinking about our own leadership options, we recognised the need to be inclusive and collaborative, agile and innovative, and show compassion and understanding to others who would also be scared about the scale of threats and change. We also recognised that there was an option to do things differently to mitigate or manage some of the threats to health we could see. We discussed the inherent power of people to adapt, find other ways of “being,” take collective action resulting in change for the good, and care for one another when supported and empowered. The technological and digital advances open to us to address health challenges were also a source of hope. While many of us expressed frustration with the speed of adoption of these within our systems, we also recognised that we are in the early phases of a digital revolution which could lead to transformative approaches to promoting health and managing illness and disease. The need to think differently about the workforce of the future was also a recurring theme. Our health services are designed as “illness” services, focussing on fixing things once they are broken. We played out thought experiments on how to flip this on its head – what would it look like if we focused on preventing and managing as well as fixing and curing? And what sort of “health” workforce would we need to invest in to achieve this “left” shift? So, it’s all very interesting, I hear you say, but how has it changed things for me and the communities I serve? I am a medical director of a mental health trust and an academic at a school of medicine, and I have found the experience of the Sciana Fellowship transformative. That has been visible in my work in the UK. I have been able to introduce new ideas into conversations in the organisations I work in and with, develop teaching sessions for students, health care workers, and leaders, and introduce concepts I have been exploring. I have used the Fellowship to expand my own sphere of impact, strengthening systems, community, and academic relationships across disciplines. I have promoted concepts of health literacy, One Health transformation, and digital innovation in multiple settings. I have gained the confidence to challenge complacency about climate change and promoted and supported the work on our NHS Green Plan and community transformation programmes, including working on bids to radically shift the delivery of mental health alongside physical health interventions at the community level. I increasingly recognise the importance of talking to and working with the vast number of people interested in health inequalities, population health, the social and commercial determinants of health, and adverse weather conditions, and I am participating with more vigour and impact in research and change programmes relating to these themes. I have been able to share my successes and challenges within the wider Sciana Fellows group, and the conversations I have had with others on the programme have inspired and enriched me, enabling me to do the same to others back at home. It has also been a pleasure to support other Salzburg Global Fellows, and I look forward to continuing to support other programmes. In short, it has been a privilege and a worthwhile endeavour, which has resulted in many new friends and allies whose power will sustain me for years to come.
