MARIETTA SPORTS MEDICINE

The Marietta High School sports medicine program is dedicated to providing the
highest quality health care for all MCS athletes.  We provide preventative care,
injury evaluations, immediate injury care, and rehabilitative services for all
injuries.  All athletes should report any injury to the athletic trainer as soon
as possible.  In the event an athlete cannot participate in practices due to an
injury they will be expected to report to the athletic training room for rehab.

The MHS sports medicine program also provides an opportunity for high school
students to gain education in medical care by participating in the student
athletic trainer program. MHS student athletic training aides assist in practice
and game preparation, taping, and basic first aid.

DO YOU HAVE EVERYTHING DONE TO PLAY SPORTS?

All athletes must have a valid MCS physical (physical and all signatures must be
done on of after 4/1/23) on file and online forms in ATS must be completed prior
to participating in any team workouts.

2023-2024 PHYSICAL

All athletes must have a valid MCS physical on file prior to participating in
any team workouts.  Original documents must be competed and turned in to the MCS
Sports Medicine program.  Forms cannot be uploaded or emailed in.  

The below form is for the 2023-2024 school year.  In order to be valid, the
physical and all signatures MUST be done on or after 4/1/2023.  Note:  This is a
fillable PDF file and should not be opened in any other program as it can
distort the file.

ONLINE ATHLETE PORTAL



Every student-athlete must complete all steps of the online athlete portal WITH
A PARENT yearly (on or after 4/1)  to be eligible to participate in any MCS
sports activity. Please be sure you have an active physical for the
2023-2024 school year.

Click here for the INSTRUCTIONS (Spanish Version) and CAREFULLY follow the
directions COMPLETELY through PAGE 3.  The portal is designed to be filled out
on a computer and will not work well on a cell phone.

PURCHASE HEALTH INSURANCE

MCS requires every athlete has health insurance to participate in any athletics
activities.

Purchase 24 hour coverage in order to participate in summer workouts next
summer.
