MISSION

YAI believes in seeing beyond disability. We are driven by our mission of
living, loving, working, and learning. Our person-centered approach positions us
to empower the people we support to reach their goals, both big and small, every
day. YAI has grown into one of New York State’s largest social services
nonprofits. Our staff of 4,000 provides supports and services to thousands of
children and adults with intellectual and developmental disabilities (I/DD) and
their families.

EMPLOYMENT SERVICES

Employment services provide job skills training, resume writing, interview
preparation, application help, intensive job coaching, and follow along support
to people with I/DD who wish to obtain and maintain competitive employment.

EDUCATION

Manhattan Star Academy - Manhattan Star Academy is a school for 5- to
12-year-olds, driven by the belief that every child has the capacity to learn,
grow, and contribute.

iHOPE - The International Academy of Hope serves children, adolescents, and
young adults from ages 5 to 21 years, who have sustained acquired brain injuries
or other brain-based disorders and who cannot be served in their local school
systems.

HEALTH SERVICES

Premier HealthCare offers primary care and specialty outpatient services to
patients who have intellectual and developmental disabilities. At our five New
York City locations, we offer outpatient healthcare services that include
primary care, dentistry, pediatrics, psychiatry, podiatry, neurology, and
ophthalmology.

FAMILY SERVICES

Family services include respite, crisis intervention, recreation, and
socialization services for families of children and adults with disabilities
living at home.

COMMUNITY HABILITATION

Specialists provide individualized skills training in the home and the
community. We focus on a person's strengths and critical skills for development,
including building skills and independence in the areas of self-care, social
skills, money management, safety, household tasks, participating in local
activities, and job success.

DAY SERVICES

The emphasis of day habilitation is participation in the larger community. Our
services include tutoring to help improve basic academic skills and
volunteering, among other activities. Participants have daily opportunities to
develop skills, including safe travel using public transportation and community
integration.

RESIDENTIAL SERVICES

Residential services are offered to people in need of group homes and supportive
apartments. YAI’s individualized residential alternatives (IRAs) provide varying
levels of staff support and supervision. Supervised IRAs offer 24-hour staff
presence and supportive IRAs offer need-based supports and services to people
who are able to live more independently.

INFORMATION AND REFERRAL SERVICES

LINK, YAI’s information and referral service, provides the latest information on
programs that make a difference in the lives of people with intellectual and
developmental disabilities and their families. LINK can be contacted by phone at

SOCIAL LIFE, RECREATION, CAMPING, AND TRAVEL

YAI offers social opportunities and events across all age ranges and locations.
Travel opportunities for adults with I/DD include weekend trips and extended
vacations with domestic and international destinations. YAI also offers an
award-winning inclusive camping experience in the Catskills to children and
teens with developmental and learning disabilities, ages 8 and up.

These seven goals are detailed in YAI’s Strategic Plan for 2018-2020. By
implementing these goals, YAI is positioning itself to be an exceptionally
important asset in the world of managed care and value-based performance.

WHAT ARE THE ORGANIZATION'S KEY STRATEGIES FOR MAKING THIS HAPPEN?

YAI provides innovative services to enhance the lives of people in the I/DD
community. Our hundreds of person-centered programs serve people of all ages and
include newborn and early childhood assessments; schools; housing; day programs;
job coaching and placement; primary, specialty, and behavioral healthcare;
crisis intervention; arts and culture programs and workshops; summer camp,
socialization programs, and recreational opportunities; and hospice.

WHAT ARE THE ORGANIZATION'S CAPABILITIES FOR DOING THIS?

As the I/DD field navigates the new world of CCOs and managed care, YAI has been
flexible and responsive to the call for new service models and payment
structures. YAI’s commitment to realizing its goals represents our promise to
the people we support and their families that, no matter what, we will continue
to provide services of the highest caliber that are built around our mission of
creating opportunities for living, loving, working, and learning.

WHAT HAVE THEY ACCOMPLISHED SO FAR AND WHAT'S NEXT?

Today, YAI has a team of more than 4,000 employees who support more than 20,000
people in the I/DD community each year. Operating throughout Downstate New York
and Northern New Jersey, YAI now offers more than 300 programs for people of all
ages.

Internationally, our activity is steadily increasing. We will continue to
leverage YAI’s consultative status with the United Nations to help others
understand our person-centered approach to seeing beyond disability. We intend
to continue to evolve in ways that enable us to offer the most innovative
services possible to people of all abilities, across the globe.
