<p>CitizenCard verifies the identity of every applicant. We have strict procedures in place to ensure the applicant's name, date of birth and appearance are checked and confirmed with the referee before a CitizenCard is issued. The
process is audited by members of the Age Check Certification Scheme on behalf of the Proof of Age Standards Scheme. <a href="https://www.citizencard.com/trusted-proof-of-age" target="_blank">Read more</a> about CitizenCard's

<p>A referee validating documents needs to complete the Digital Referee Declaration Form - state which original document(s) confirming the applicant's identity they have seen, state the applicant's name and date of birth according to
those documents, provide their details such as name, job title, work address, work contact details and consent to be contacted at their place of work.</p>
<p>Once the Digital Referee Declaration Form is submitted, the answers provided will be checked using online databases and the second stage will start - the referee will be contacted directly by CitizenCard to verify the applicant's
identity.</p>

DIGITAL REFEREE DECLARATION FORM: REFEREE VALIDATING DOCUMENTS APPLICATION VERIFICATION PROCESS: PART 1/2 Applicant Joe Ward, who provided you with a link to this form, completed an online application for a CitizenCard ID card and needs a referee. A referee is a professional person who will confirm the applicant's identity. The referee needs to complete the Digital Referee Declaration Form below and answer all questions only once they have met with the applicant and seen original document(s) confirming their identity. The applicant needs to consent to the referee confirming their personal details either verbally or in writing. Once CitizenCard receives the submitted information the verification process will start. More information INFORMATION ABOUT A REFEREE VALIDATING DOCUMENTS CitizenCard verifies the identity of every applicant. We have strict procedures in place to ensure the applicant's name, date of birth and appearance are checked and confirmed with the referee before a CitizenCard is issued. The process is audited by members of the Age Check Certification Scheme on behalf of the Proof of Age Standards Scheme. Read more about CitizenCard's verification process. A referee validating documents needs to complete the Digital Referee Declaration Form - state which original document(s) confirming the applicant's identity they have seen, state the applicant's name and date of birth according to those documents, provide their details such as name, job title, work address, work contact details and consent to be contacted at their place of work. Once the Digital Referee Declaration Form is submitted, the answers provided will be checked using online databases and the second stage will start - the referee will be contacted directly by CitizenCard to verify the applicant's identity. A PROFESSIONAL VALIDATING DOCUMENTS NEEDS TO SEE AN ORIGINAL DOCUMENT(S) CONFIRMING THE APPLICANT'S IDENTITY. Depending on whether a referee knows the applicant or not we accept different documents. If a referee knows the applicant personally we accept the following documents: * Adoption certificate * Birth certificate * Certified copy of a birth certificate * UK Marriage certificate which states applicant's date of birth * Disclosure and Barring Service certificate (DBS) * Global or European Health Insurance Card * NHS Medical Card * Gender Recognition Certificate * UK Naturalisation Certificate * Passport (UK or International; must bear a recent likeness to the applicant) * Photo Driving Licence (UK or European; must bear a recent likeness to the applicant) * European National ID card (must bear a recent likeness to the applicant) * Biometric Home Office photo ID * Proof of Age Standards Scheme card * Ministry of Defence Form 90 (Defence Identity Card) If a referee doesn't know the applicant the following photographic ID documents can be used: * Passport (UK or International; must bear a recent likeness to the applicant) * Photo Driving Licence (UK or European; must bear a recent likeness to the applicant) * European National ID card (must bear a recent likeness to the applicant) * Biometric Home Office photo ID * Proof of Age Standards Scheme card * Ministry of Defence Form 90 (Defence Identity Card) In addition, if the applicant has changed their name since birth and the document shows their old name, a referee needs to see an original document(s) confirming their name change.
