Sentara Martha Jefferson Healthwise: Cancer survivorship plans

ALBEMARLE COUNTY, Va. (CBS19 NEWS) -- A cancer survivorship plan is an 8- to
10-page document that provides survivors with items like their treatment
history, possible side effects, what medications they need to take and when,
when they need to follow up with screenings, how often they should see their
oncologist, or when they should see the survivorship clinic nurse practitioner.



"It also gives them some tips and tricks of, they're still going to have some
late side effects from treatment, how do we deal with those, its also going to
help them with their emotional needs, and what resources are out there for
them?" said Danielle Schrader, Blue Ridge Cancer Program manager.



The survivorship care plans are given to survivors about three to six months
after they finish treatment. It is tailored to the patient by the nurse
navigator, who meets with the patient and goes over the packet with them.



"I think it does make it less stressful for our patients, the cancer journey
doesn't end when treatment stops or is done, there is still a lot of anxiety and
patients are fearful when they go into survivorship, is this going to come back,
what do i need to do, how do i keep this from happening again, so i think the
survivorship care plans gives that path of knowing what they need to do and how
frequently they should be following up," said Schrader.



There are three nurse navigators at Sentara Martha Jefferson Hospital, including
one specific to breast cancer survivors, one that works with all kinds of cancer
survivors and one that works with head, neck and lung cancer survivors. These
navigators stick with a patient from diagnosis through survivorship.



The survivorship care plans they help develop help patients on their journeys to
healing, even afer their treatments stop.



"People feel, oh my treatment's done, I'm better now, I can go back to the way
things were and you still have to give your body time to heal and make sure
you're taking those periods of rest when needed and taking care of your
emotional wellbeing as well," said Schrader.



The goal is for patients to transition to the survivorship clinic once they
complete their treatment.
