SPORTS PHYSICALS

Player Physicals will need to be completed prior to taking the field on the
first day of practice. If you do not have the signed, league physical, your
player will not practice (applies for both football and cheer).

LEAGUE PHYSICAL FORM

INYFC Physical Fitness & Medical History Form PAGE 1 of the INYFC form is
REQUIRED. Physicals are good for two years from the date of the doctor's
signature. Chart notes are NOT accepted.

The following documents will be needed to get your child verified through
NSID:  

Current Photo,
Birth Certificate,
School Verification Form
This form MUST show the current school year, grade, child's name and name of
school.
If your child is homeschooled and does not have a report card, please provide a
letter from parent with athlete’s name, date of birth, grade attending, address
and name of homeschool affiliation. This letter should be signed with a
statement that the information provided is in fact true.

   above and beyond to implement precautionary measures, creating an environment
   where everyone can enjoy the game without compromising safety.
