COVID-19 CAREGIVER VACCINATION RECORD

PLEASE LOGIN TO PROVIDE INFORMATION REGARDING YOUR VACCINATION

This tool is Single Sign On (SSO) enabled which will require UPN (User Principal
Name).

Example UPN Formats:
first.last@providence.orgfirst.last@stjoe.orgusername@providence.orgusername@stjoe.org

To login, all users need to use their 'modern' network ID (known as a UPN) which
will normally end with @providence.org or @stjoe.org.

This is also true for legacy SJH or Providence caregivers who don't have a
Providence provided email address.
