          <p><b>Why is this questionnaire being administered? </b></p>
          <p>The purpose of this questionnaire is to gather your input on the tasks performed, competencies required, and training needs of those leading in a hybrid work environment, across HHS. A hybrid work environment is defined as a flexible
            work model that supports a blend of in-office, telework, remote work, mobile work and/or dispersed teams. The information collected may be used to support the training and development of leaders in a hybrid work environment across HHS.

          <p><b>Why is my participation important?&nbsp;</b></p>
          <p>The U.S. Health and Human Services (HHS) would like input from leaders to guide decisions on how best to develop the workforce of those leading in a hybrid work environment. Your participation in the questionnaire will directly influence
            these decisions. Your input is also essential for ensuring the results accurately reflect the tasks performed, and the competencies and skills of leaders in a hybrid work environment.&nbsp;</p>

          <p><b>What information will I be asked to provide?&nbsp;</b></p>The questionnaire will ask you to provide ratings such as the level of importance about job-related tasks and competencies.&nbsp; <p></p>
          <p>The questionnaire contains four sections:&nbsp;</p>
          <ol>
            <li>Background Information</li>
            <li>Task Ratings</li>
            <li>Competency Ratings</li>
            <li>Proficiency Ratings</li>
          </ol>

          <p><b>What will happen with my responses? </b></p>
          <p>All responses to this questionnaire will be strictly confidential. HHS has partnered with the U.S. Office of Personnel Management (OPM) to support this initiative. OPM will administer the questionnaire and analyze the results for HHS.
            OPM will combine your individual responses with responses from other survey takers and will report results in aggregate. No one at HHS or your agency will have access to your individual responses. The information collected will be
            reviewed and analyzed by OPM Human Resource personnel, agreement, or contractor support personnel. </p>

          <p><b>How long will it take to complete the questionnaire? </b></p>
          <p>The time it takes can vary by individual. We estimate this questionnaire should take about 45 minutes to complete. </p>

          <p><b>Please consider the following as you complete the questionnaire: </b></p>

            <li>Read the instructions for each section carefully. </li>
            <li>Answer all questions as honestly and accurately as you can.&nbsp; </li>

WELCOME TO THE U.S. HEALTH AND HUMAN SERVICES HYBRID WORK COMPETENCY ANALYSIS
FOR LEADERS QUESTIONNAIRE

Why is this questionnaire being administered?
