    <p class="body-content"> These pages are used to report the death of a retiree or survivor annuitant who is currently receiving a benefit from OPM under the Civil Service Retirement System (CSRS) or the Federal Employees Retirement System (FERS).
      Please fill in all of the appropriate data on the following screens. Once completed, the data will be submitted electronically. Form fields which are required will be identified with a red asterisk and the word "required" ( <span

REPORT ANNUITANT DEATH

These pages are used to report the death of a retiree or survivor annuitant who
is currently receiving a benefit from OPM under the Civil Service Retirement
