What is 100Plus?

100Plus is an ecosystem of remote patient monitoring devices for patients on
Medicare. The platform solution sends patient data to medical practitioners for
review, which enables these providers to qualify for Medicare reimbursement
under the new Remote Patient Monitoring program. As part of the Remote Patient
Monitoring program, providers are reimbursed for the setup, hardware, and
service costs, as well as ongoing review of patient data. Eligible Medicare
patients receive devices at no cost1 for them.

What is Remote Patient Monitoring (RPM)?

Medicare has long attempted to create incentives to providers to engage with
elderly patients more proactively, versus traditional periodic visits when a
patient’s condition has escalated. To drive this initiative forward, 2019
Medicare's Remote Patient Monitoring program compensates providers for
recommending and reviewing data from products like 100Plus that monitor
patients. Remote patient monitoring simply refers to the use of technology to
record health data for review by a provider in a different location than the
patient.


How do I get 100Plus remote monitoring devices for my patients?

If you have identified a Medicare patient with at least one chronic condition
and obtained their consent to participate in our remote patient monitoring
program, you can enroll the patient via the 100Plus portal. It’s a simple 3-step
process:

1. Enter their basic info (name, address, etc).
2. Have the patient sign the 100Plus User Agreement.
3. The 100Plus device(s) will be shipped to the patient; the device will be
ready to use with cellular connectivity enabled.

The device will be associated with the medical provider who recommended it. Your
patient can simply open the package and start to use it.It’s required that you
obtain consent from your patients to provide remote patient monitoring services
and important that you discuss your recommendation and the program with your
patient so they understand its benefits and the importance of their continued
usage of the device once they receive it.These steps are simple — we will show
you how to do it during the remote training session. We also offer end to end
enrollment services where we do most of the onboarding for you.

Where can I track my patients’ activity?

The 100Plus portal contains all the information you need to monitor your
patients’ activity. You can track patient enrollment, see if they have provided
all the necessary information to receive a device, including signing the User
Agreement, and track their device shipment. Once they start using their device,
you can also see all of their clinical data.

How do I get reimbursed?

Medicare reimbursement rules compensate medical providers for prescribing and
reviewing data produced by remote patient monitoring devices that capture,
store, and transmit patients’ physiological data and subsidize the device and
service costs.

Every month, you receive up to $242 per patient per month, or $2900 per patient
per year, for CPT 99454, 99457, and 99458 (2x). In addition, you receive a
one-time reimbursement for setup and patient education of up to $24.98 per
patient2.
