HOW TO CLAIM YOUR PRIZE – 3 EASY STEPS!

 1. All winners must provide their original winning ticket as well as their
    name, address, phone number and 1 piece of government-issued photo ID to
    Trillium Health Partners Foundation to claim their prize. Please note that a
    health card is not an acceptable piece of identification. 
 2. Contact Trillium Health Partners Foundation at winforhealth5050@thp.ca or at
    1 (905) 848-7575 to arrange a date and time to come in person to the
    Trillium Health Partners Foundation office at 2085 Hurontario Street, Suite
    401, Mississauga ON L5A 4G1 (first street north of the QEW on the east side)
    to complete the prize claim process.
 3. Once at the office, please show the winning ticket and government-issued ID
    to our staffer and complete a Winner’s Form with your personal information.

The prize payout by cheque is mailed and may take 4-6 weeks to arrive. All
prizes must be claimed within 6 months of the relevant draw, unless otherwise
specified by Trillium Health Partners Foundation.
