RULES AND REQUIREMENTS:

 * Parent signing in must leave a picture ID with us and is the only person
   permitted to sign the child/children out.
 * Parents need to complete registration packet (includes rules, parent/child
   info & waiver) and provide up to date shot records on FIRST visit.
 * Registration packet must be renewed every 2 years and shot records need to be
   updated at the earliest recommended age per the CDC guidelines.
 * Reservations are required. Premier/Platinum members may make a reservation 48
   hours in advance if space is available. Please note that reservations will be
   cancelled after 15 minutes if you do not show up.
 * Children may not stay longer than 2 hours per day. Visits are limited to one
   visit per day.
 * ALL children must have socks on regardless of age.
 * Children may bring water ONLY. Food, juice, milk, flavored water, etc… are
   not allowed on the PlayRoom floor.
 * Parent that signs child/children in must stay on the premises while children
   are in our care.
 * Children ages 6 weeks to 13 years are welcome in the LVAC PlayRoom.
