RADIANT HOME CARE


Radiant Home Care was established in 2017 in response to local community demand
for quality disability support services. The Director and Management Team are
highly experienced in the care sector. Collectively, the Radiant Home Care team
find providing care to the community a very rewarding experience, with the staff
being treated as family – clients who have so much gratefulness towards the
Carers who really enjoy what they are doing, and this personalisation cements
their part within the community.






HELPING YOU ON YOUR NDIS JOURNEY


Navigating the NDIS can be difficult. We’re here to make things easier. We work
closely with NDIS participants to help them build their capacity and achieve
their goals, regardless of disability. As a registered NDIS provider in
Melbourne & Adelaide, we are able to support people living with disabilities and
help them to access the services they need. Accessibility, respect, and dignity
are at the heart of everything we do.

We match you with a carer that is perfectly suited for your needs and who you
can get along with. Each member of our team is highly trained and able to
provide the level of support you require. We strive to help each of our clients
achieve their personal goals and improve their quality of life through our

WHAT'S YOUR NDIS PLAN?


All NDIS participants must create a plan that details their goals, existing
supports that are available to them, and the disability services and support
that they need in the future to achieve those goals. To be a participant, you
must have a disability that is significant and permanent.

Each of our clients is different, meaning that their goals vary widely. Some
want to build the capacity to live in their own home, while others want to focus
on building social skills or training and employment goals. Whatever your
aspirations are, we’re here to help you create opportunities and help you fully
participate in life.

Need help with your NDIS plan or any other aspects of the program? We can
support people through the process and help them access the most helpful
disability services for their circumstances.

We offer a wide range of disability services in Melbourne & Adelaide. As a
recognised NDIS provider, you can trust that you will get the best quality

PARTICIPATE COMMUNITY (SOCIAL ACTIVITIES)

By helping you build meaningful relationships and become more involved in the
community through fun social activities, you will be able to develop essential
social skills and connections. We take on board any hobbies or preferences you
may have – or, if you're unsure, we can help you find a new one!

HOUSEHOLD TASKS

Need help around the house? We can help with any cooking, cleaning, gardening,
or other chores that need to be done to ensure that you can live independently.

ACCOMMODATION/TENANCY

Living on your own can be tricky. Our team can help you find accommodation and
navigate the complexities of the rental system so that you can choose a place to
live and stay there.

INNOVATIVE COMMUNITY PARTICIPATION

If you require something a little different from our regular community
participation service, we can help find a way to engage with the community in a
way that suits you.

GROUP/CENTRE ACTIVITIES

Group activities are a great environment for those with a disability to build a
number of important skills and engage with others. We can help you get involved,
either embracing a passion you already have or helping you find something new to
try.

PERSONAL ACTIVITIES

For clients who require a little more support and medical care, we have a team
of nurses and carers who are able to provide a higher level of personal care.
This includes things such as bowel care, enteral feeding, wound management and
more.

SUPPORTED INDEPENDENT LIVING

Our team is dedicated to helping you build the necessary skills to live on your
own. We provide supervision and help with daily tasks to ensure that you can
reach your goals and create the life you want.

SHORT & MEDIUM TERM ACCOMMODATION

If you need somewhere to stay for a short while that supports your needs or are
in a transitional phase of your life, we can provide accommodation for up to 90
days.

ASSIST PERSONAL ACTIVITIES

If you need a little support with everyday activities – personal hygiene,
mobility, feeding, and more – our carers can provide assistance so that you can
focus on other things in your life.

ASSIST-TRAVEL/TRANSPORT

We get you where you need to go, ensuring that you are still able to participate
in the activities you find meaningful and spend time with others. Whether you
need to get to an appointment or you are going on a trip, we help secure
accessible transport that suits your requirements.
