HELP IS NEAR Our mission at Help Is Near LLC is to provide comprehensive support to individuals facing housing challenges amongst other challenges. We believe that collaborating with experienced case managers like yourself can significantly enhance our ability to serve the community effectively. By combining our strengths, we can empower individuals to achieve housing stability and rebuild their lives. We are based in Minnesota and assist around surrounding areas. Anchor 1 OUR SERVICES Housing Stabilization Services is a Medical Assistance benefit to help people with disabilities, seniors and individuals that are at risk of homelessness, find and keep housing. Housing stabilization services are state plan (not waiver) Home and Community-Based Services (HCBS). HSS is composed of three different services: Housing Consultation, Housing Transition and Sustaining 1 HOUSING CONSULTATION Service to develop a person-centered plan for people without MA case management services. This service supports an individual in building a housing focused person-centered plan. 2 HOUSING TRANSITION Service that supports a person to find housing. 3 HOUSING SUSTAINING service that supports a person to maintain housing.
